in the world you'd
want implants in.

For me to pull the muscles
that much further,

the risk is the nerve.

My name is Whitney,
and my breasts

look like upside-down zucchini.

At the age of 14, I
developed very large breasts,

and so any time a boy
would come up to me,

it wouldn't be
to flirt with me.

It was to try to
look down my shirt.

Then one day this boy,
and he knows who he is,

nicknamed me Titney Whitney,
and it just stuck.

I got pregnant
at the age of 18,

and after I had Skyler,
my breasts fell.

They literally hung
to my belly button.

It was devastating.

The first plastic surgeon
I had went to, I told him

I would like a reduction,

and I come out with
these massive boobs.

The doctor had put breast
implants in without my
knowledge.

When I first saw my
breasts, I was shocked.

They were just as big
as they were before,

if not bigger looking
because they were so high.

And then about
three months later

I was lookin' in the mirror,

and when I would turn
to the side,

I would notice that there

was implant up here and
breast tissue down here.

And I was like, did
something go wrong?

Did I do something wrong?

So that's when we contacted
the surgeon again.

I told him, please
make me smaller,

and all he kept sayin' was
I had too much elasticity

in my skin and that it
just wasn't possible

to remove breast
tissue at this age.

And he said, well let me go
back in and do another lift.

The breasts were just uneven.

The right one was bigger
than the left one.

So we went and saw him
again for the third time,

and my breasts looked the same.

It mentally really messed me up.

I look different than every
other woman in this world.

My kids like to go run at
the track and do the bleachers.

I would love to
do them with them.

However, the bouncing
and the running,

it's just, right now,
it's just impossible.

It's almost like I
was just torn apart

at my breasts,
and I don't know why.

But it's been so long
and I've lived this way,

and I'm just prayin'
for a change.

All right, I'm
ready for a strike.

If my boobs wouldn't
get in the way.

So how you feelin' about it?

If I'm able to get surgery
by Dr. Dubrow and Dr. Nassif,

I will be able to go play
outside with my kids more.

Mentally, physically,
it would change my life.

My name is Lisa, and my
surgeon gave me a cobra neck.

It has been a while,
hasn't it?
Yeah.

I grew up outside of
Somerville, Texas.
We called it the sticks.

It was a lot of fun growin'
up way out in the country.

I rode horses probably
almost every day.

I mean, it was my passion.

My first husband I met
actually when I was 14,

and we didn't do anything,
didn't go anywhere.

It was a really
lonely relationship,

kinda like just existing,
you know, for a long time.

I don't even think he ever even
told me I was pretty. [laughs]

Comin' out of my first marriage,

I was really excited
to start my life

'cause I felt like I
didn't really have a life.

Paul and I met each other
when I was 37, and he was 22.

I worried about the age
difference more than he did.

The next thing we knew, eight
years had already passed by

and we were still together,
and we got married.

I couldn't ask for a better
husband, partner, friend.

Paul bein' so much
younger than me,

I knew that I wouldn't be
able to carry a child for him,

and I knew that he would
be a wonderful dad,

so we started
searching out to find

a surrogate to carry
a child for us.

We had Madison in 2008,
and then in 2012

the same surrogate
had Marshall for us.

After having Marshall,
I did have more people

start askin' me if I was
the kids' grandmother,

and then it made me
start lookin' at myself.

I mean, I know I'm old
enough to be a grandmother,

but I didn't want to
look like a grandma.

I went to a plastic
surgeon to see

about getting my face
corrected.

I asked him what he thought
I needed to have done,

and he said, deep plane facelift

is gonna last you
about 20 years,

and I thought,
well that's great.

That way I can just age
out with Paul now.

After surgery I did
get a real big hematoma

on the left side behind
my ear and my neck.

It took several
weeks for even that

to go down 'cause
it was so large.

A week after the surgery I
could already feel that things

were pulling loose under my
chin, and then as time went on,

it just kept falling more
and more, and then you

could actually see that
I had the cobra neck.

My daughter said, mom, it looks

like your neck is falling down.

I went back to see my doctor.

He goes, I didn't do
the full facelift cut.

He just said, well I just
felt like I didn't need to.

I thought he would've been
more sympathetic towards me,

but I guess he just didn't
wanna admit that he was wrong.

I took the kids to
school the other day,

and it was a different
teacher at school.

She's like, are you
the kids' grandmother?

[Paul laughs]
And I was like, no, I'm the mom.

I know it's there,
so I feel like

everybody else is looking at me.

[Paul] Yeah.
You know.

What bothers me
the most about the results
is how much it bothers her.

She's just not the same
person she used to be.

She was so outgoing,
and after the surgery,

she's really withdrawn,
really sensitive

as to what people think about
her, and I hate seeing that.

I just want her to be happy.

I want her to see what I see
when she looks in the mirror

and get back out there and be
the Lisa that she used to be.

I'm proud of you.
I'm proud of the kids.

Y'all are everything to me.

But I didn't pay $13,000
to have me look older

instead of lookin'
a little bit younger.

That's a big deal.

It has taken me a while
to have the problems

that I have fixed
because I'm scared.

I'm scared that somebody
else might mess up.

But I do wanna come out of
the shadows and try to enjoy

doing things again with
my husband and my kids.

Getting my face corrected
would give me the confidence

that I need again to get back
out there and enjoy life.

and sometimes I can't.

I went in for a
reduction and a lift,

and I came out with implants.

Why does a girl who's
so thin have so much

lax breast tissue?

Gosh, when I was in the
eighth grade, I had double Ds.

They called me Titney Whitney.

Girls would ask me what I
would do to get big breasts

so I told 'em I ate
a lot of carrots.
[Cole laughs]

And every girl in eighth grade
was walkin' around orange

for the next two years.
Right.

Chompin' 'em down.

Physiologically, carrots don't
cause enlargement of breasts.

However, they do allow
you to see them better.

Yes, mm-hmm.
That explains

why the tissue is so lax,
but you have breast implants,

so how did that?
Well, okay,

I had a child.

They immediately fell, when
I say, past my belly button.

Was there volume in the breasts,

or they just started sagging?
No.

Empty bags hanging.
They were just empty bags.

So usually when you
don't have large breasts

and you have one child, they
deflate but they don't droop.

Right.
If you start out

with very large breasts,
they deflate

and they droop.
Fall.

Then I had my plastic surgery.
Okay.

I went in for a
reduction and a lift,

and I came out with implants.
Wait, whoa, whoa, whoa.

Did they tell you they
were gonna do implants?

He said it might be
a possibility,

but I said lean as much
far away from it as you can.

I would like a reduction.

And he didn't take
out breast tissue

because he said I was too young.

He failed to mention
that before the surgery.

What they end up looking like?

Massive.
They were up.

[Dr. Dubrow]
They were up right away?

Yes, and they were uneven.

Right, so the second operation
was done for unevenness.

Yes, sir.
And they got 'em together
and more even, but then--

More even, but then
the sagginess started,

and so my nipples
started falling.

So he took me in again.

My mom thought I was crazy
for lettin' him operate on me.

So what did he do, third time?

He kept calling it
"skin removal,"

like you know, the um--

Did he adjust
the implant at all?

He said he didn't
touch the implants.

So these are the
original implants?

Yes, sir.
Okay, so what's goin' on now?

I mean, we saw the pictures.
You have it again.

You have implants up here.
Yeah, I have it again.

Your breast tissue's
hanging off them.

I just want 'em perky
and as little as--

Normal lookin'.

So leave the implants alone

and reduce the breast tissue?

It's like a puzzle.

It is, there's a
lot of moving parts.

I think we need to go
in the exam room...

[Whitney] Yeah.
and figure them out.

[Dr. Dubrow] Unfortunately,
Whitney kinda fell victim

to the classic plastic
surgery mistake,

and that is to take a large,
deflated, droopy breast

and treat it by putting
a very large implant in it,

thinking that you can restore
it back to the way it was.

It never works.

It always upsets the patient.

Okay, do me a favor.
Put your hands here and flex.

Yeah, they're definitely
on the muscle,

and the muscle
is pushing them up.

And they're pretty soft.

So it's pretty clear
that your pectoralis muscle,

which the implants are under,
is holding them up.

[Whitney] Okay.

So if you go in there and
just release the pectoralis

muscle on both sides,
they'll go boop!

They'll drop right into
the envelope,
which is a good thing

because if the implant
and the breast envelope
are together,

you'll never have
that dissociation.

Unless you suffer capsular
contracture, which you

don't seem to suffer,
which is scar tissue formation.

Here's the truth.

I think your surgeon
didn't really understand

the fact that
the implants are being

shelved up and held up
by the muscle.

No matter how many times you try

to lift up
the breast tissue to meet

them, it's not gonna work.
Right, it's not gonna work.

Right.
But you know what?

If we downsize it too much,

they're gonna be
very pancake-y, and then

you trade one problem
for another. Right?
No, I don't want them pancake-y.

Whitney's problem
is deceptively difficult

because she has
a certain basic width

to her breasts,
which you can't alter.

So if you go too small,
she'll lose projection.

Yeah.
This is me.

That's what you want.
You got it?

So I'm hoping to
make her small enough

to achieve her goals
but large enough

to have a good-looking breast.

So I gotta go
downsize the implant,

pocket closure, release muscle,

tighten the envelope,
I mean it's everything.

So I guess what we're
looking for is for you

to be on the same page
in terms of the fact

that we can't make them as
small as you ideally might

wanna be.

I feel a little
disappointed that I

can't go as small as
maybe I had in my head,

but we have to do
what's best for my size.

she had some face work, but

a textbook error,

I was 22 when we met.

So you had a facelift.

Did you have any improvement,
or is it worse?
No.

Oh no, it's completely worse.

Actually, within three weeks
I knew somethin' was wrong.

As more of the
swelling went down,

I could see my jowls
comin' back here.

[Dr. Nassif] Okay.
And then--

These bands.
I call 'em waddles.

I couldn't open my mouth.
I couldn't, I mean I couldn't

even bite a sandwich
at that time.

So when I went back
to talk to him about it,

he goes, Lisa,
he goes, "I did nothin' wrong."

And he shows me these pictures,
my before and after,

he goes, "Look, I made you
look 10 years younger."

And I'm like,
"That's my before picture."

He picked up the before
photo and did that?

Yes, and then he didn't
know what to say.

After Lisa came out of surgery,
you could see the difference

in the actual surgery,
what she had done.

And you could see the
difference in her personality.

So it was definitely
a mental impact.

It was a big blow.

I've gone to five
different plastic surgeons,

and they've all said that
this would be a difficult thing

to fix because of
the way it dips in.

This is actually one of the most
difficult things to correct.

Uh-huh.

After you've had surgery.

Lisa has what we call
a cobra neck deformity.

That's when the platysmal
bands are very prominent,

and they look more prominent
because all the tissue

in the midline has been removed,

and that's
the hollow part of it.

When you have a problem like
that it's very interesting

number one, and two,
it's very difficult to fix.

How is all this
affecting your life?

I try to stay away from public
as much as possible.

[Paul]
I think she's so self-conscious

about how she looks now,
after the surgery,

that she just kinda stays
at the house a lot more.

Do you feel this has
caused some depression?

Oh yeah, yeah.

[Dr. Dubrow]
I'm glad you're with us.

Me too.
So that's why

we need an exam in here.

We have to really see
what we need to do.

[Lisa] Okay.
So let's go in the exam room.

Okay.

I'm very self-conscious
about the way my neck looks
right now.

I look downwards
to not show my neck.

I just wanna walk around
with my head held high.

I want the confidence
back that I've lost.

Smile for us.

Look at how far that is.

Boy that really is
sucked all the way in.

Feels like it's been stitched
all the way in there.

Can you put your head
back a little bit for us?

Oh yeah.
Oh yeah.

[Dr. Nassif] Oh there's
some scar right there.

Looks like a lipo scar
was over-suctioned.

Lisa really has
an unfortunate result

of a poorly executed operation.

Way too much fat was removed.

You can almost see
liposuction grooves

from the cannula in
the neck and chin area.

This is a much more
significant operation

than the normal
full facelift is.

It appears that your muscles

were already kind
of separated before,

but there was some fat on them.

And when there's
some fat on them,

Camouflages them.
we can't really
see it that much.

Right, right.

So we think that he did
some aggressive liposuction

in the area, and now you
have that over-suctioned,

hollow area called
the cobra neck deformity.

You know, as it comes out,
the two edges

of it that are prominent,
that's your bands.

I may wanna put
a little fat in there.

Okay.
And might have to
take a little bit

of fat from your belly.

That'd be okay.
Just a little bit there.

After we do that,
we gotta play with the muscle

to try to get it
as tight as we can.

The issue with this
is even after all
that beautiful work,

the muscles could pull apart.

You might be back
to where you were.

There's always risks in
regards to doing a facelift,

especially a revision facelift.

So what we have to do is Lisa
and I have to work together

as a team and God willing
give her a beautiful result

that's gonna make
her look younger

so that Paul and her look
like husband and wife.

After we do this,
we do you wanna try to extend

around your hair in the front

and give you more
of a vertical lift.

Mm-hmm.

So another deep plane facelift.

Okay?

So in essence what
you're having is a revision

neck and face lift with
some type of volumization

of this middle part,
of what we call the submentum.

Does that make sense?

Scary stuff, right?
I don't think so.

I don't think so, I mean, no.

Heart surgery.

You got the right person
to do this, though.

I'm willing to take
the risk of that,

and I know I'm in good hands.

Bottom line,
coming out of a facelift

and looking older as
compared to when you

came in, that's unacceptable.

We have to be very careful,
because if I go a little

too deep, I'm gonna
injure some of the nerves.

Well how are ya?

Let's get this.
Feeling a little bit nervous.

It's normal to be nervous.

This is an extensive
revision face and neck lift.

We just don't know yet
what's gonna be the best way

to fix it, so I'm
gonna use everything

in my little toolbox
to put this together,

but it's gonna be really
more of a wait and see.

Okay.

My last surgery wasn't
a good experience at all,

and my biggest fear
is that this surgery
won't get me fixed.

So the doctor did do what we
call a platysmal ligation.

He took these muscles
and pushed 'em together.

However, your muscles
[makes ripping sound]
pulled apart.

Okay.
For me to pull the
muscles that much further,

the risk is the nerve.

There is a chance that you
could come out with one lip

not depressing at all
and look lopsided.

My coronary arteries are
gonna spasm a little bit.

That's my arteries in my heart.

Okay. Okay.
As I'm doin' this.

Cobra neck deformities
are not just simple fixes.

Lisa's doctor took too much fat

out of the subplatysmal area.

My goal is to just really
try to do a good heightening

to cover up and fill in
that hole in her neck.

How's it gonna work?
I don't know until
I get in there.

[Dr. Nassif]
For Lisa's surgery today,
I will begin by opening up

her previous incision
under her chin

and performing
a corset platysmaplasty
using cadaver skin

to stitch together and
tighten the neck muscles.

Hopefully this will get rid
of the cobra neck deformity.

I will then release the blocky
incisions around the ears,

elevating the skin and
tightening the muscles.

This will give her a more
youthful, natural look.

Okay, the incision's
made into the neck.

And now we're gonna dissect down
in the area of the cobra neck
deformity.

It's gonna be a little
bit more of a challenge

just because we don't
exactly know what the anatomy

is gonna look like, so we
have to be really careful

that we don't penetrate
any of the muscles

that control movement
of the lower lip.

[suctioning]

Hold on, hold on, stop.

There's muscle.

I'm getting more bleeding.

For me to get the
platysmal muscles

mobilized, I have
to lift up the tissue,

lift up the muscles,
separate the skin.

We're going in through the
deep structures of the neck.

Yeah, it's like scarred
down the muscle.

But when you perform
a revision facelift,

we don't know how much
scar tissue there is,

so we have to be very careful.

It's dangerous.

Give me tongs when we get in.

And then cauterize and then cut.

[suctioning]

[Dr. Nassif] Okay we just
finished freeing up

the complete muscle of the
neck, all the scar tissue.

Okay we have to open
up the face next.

And we have to be very
careful because if I

go a little bit too deep,
I'm gonna injure

some of the nerves that
control the facial movement.

One of the reasons why
you have to be careful

of operating on the
face is because all

the nerves that
control the muscle.

And in doing a revision
facelift, those nerves

can be either more superficial
or more deep or in the middle

of a scar and you can't
see them all the times.

And if you damage one
of those nerves, you're

gonna walk around with a
paralyzed portion of your face.

Oh, there's a nerve.

Yeah, see, this is
just not opening up

the way I like it
to open up, you know?

This is kinda scary for me.

Right here, here's
the blood vessel, right.

We're looking for muscle.

That looks like
a platysma muscle.

We redirected all the deep
muscles and elevated them

to give her a better
look, better contour.

Now we're elevating
the neck tissue,

and we're gonna
kinda step our way up

towards the cheek area
and tighten all that up.

Okay, so the face is moving.

So we have a lot of
movement in the neck.

Now putting this stitch in,

it's going to tighten up
the neck.

Last touching on
this birthday cake

is to put the icing,
which is a little bit

of fat and the AlloDerm,
which is cadaver skin.

I like using cadaver skin

in repairing cobra
neck deformities.

The reason why is the skin is
usually stuck to the muscles,

and I need to actually add
some bulk in a separation

to give some more
fullness in there.

As long as those darn
muscles don't pull apart.

All right, so we're done.

It really looks good.
I just hope to God she heals
as beautifully

'cause that is an
incredible neck.

It's really hard to
explain when you've

lived a certain way for so long,

and you know what you
want to look like,

and you hope after today
that that's exactly what's

gonna happen, that I will
just be completely happy.

The big challenge with
you is that it's tough

to downsize you and
keep a cosmetic balance

between the projection
and the width.

I'll go as small as we can,

but I won't know
till I get in there.

Yes, sir.

I won't know how much smaller
I can make Whitney's breasts

until I release the muscle,

allow them to drop
into their pockets
so the breast tissue

and the breast
implants are together.

To be honest with
you, plastic surgery

has not done Whitney any
favors up until this point.

Let's see if I can
reverse that trend.

[Dr. Dubrow]
For Whitney's surgery today,

I will begin by making
incisions

below her areolas and
removing her implants.

I will then remove some of
her natural breast tissue
and tighten the pockets

to allow for the placement
of smaller implants.

Finally, I will perform a
breast lift on both sides,

leaving Whitney
with smaller breasts

that are proportionally
sized for her body frame.

The first maneuver is
to go into the pocket,

release the muscle,
and allow the implants

to sit where they should,
inside of the pocket.

Let's see why
it's sitting so high.

Preoperatively there
was only two things

that could cause Whitney's
breast implants to sit high.

One, inadequately
released muscle,

which requires muscle
release, or two,

capsular contracture, which
requires scar tissue removal.

Geez. Boy, she got a lot
of breast tissue, geez.

So look, it's scar tissue.

See? It's capsule.

Ouch.

Wait, wait, wait, almost.
Hold this.

Now that I'm in Whitney's
pocket and I see that the muscle

is actually not
holding things up,

and I know in fact
it's due to scar tissue,

I'm going to remove scar
tissue and not the muscle.

Okay, so all I've done
is release the implant

into the pocket
where it should be.

It's sort of just
a big, hangy breast.

This would be very easy
to maintain this size

and just tighten
up the envelope,

but that's not what she wants.

This is actually
a really tricky case

because Whitney wants to
be substantially smaller,

and if I use any
reasonably sized
breast implant,

she'll probably end up
larger than she wants to be.

So it's a fight between
whether to use implants

for projection or whether
to use her own breast tissue

because if you
don't use implants,

you may have no upper
fullness and no projection.

I mean, she's already
had a complication

from breast implants with
them shoved up into her chest.

If I can get her a
good shape and size

with no implants, she'll
never have that again.

[sighs]

Like an impossible case.

I mean, why did she
need breast implants?

I don't know, let's see.

Ugh, I don't know what to do.

[bleep].

[sighs]

Let's cut this.

Okay.

So we're gonna commit
to this, right?

I think so.

I decided to do a breast lift

using Whitney's own breast
tissue without implants.

The question is, can I
move her breast tissue

around to make her perky but
also give her the projection

that's required for
a good cosmetic outcome?

This is Whitney's breast,
lifted, reduced,
with no implant.

use breast implants,

but I firmly believe
that to achieve
the size and shape goals

that we both want,
the best course

of action was to
not use them at all.

We pulled the muscles in the
neck to give you just overall

a tighter-appearing,
you know, 25-year-old neck.

Dr. Nassif said I had
a 25-year-old neck,

and that just makes me so happy.

I was just hopin' for
a 40-year-old neck. [chuckles]

I don't have my waddles.

They're gone.

beautiful that is?

for about a good three months.

Today, Lisa looks great.

few weeks for healing

out in this area and
see how this is healing.

We wanna make sure that it
doesn't start sinking in again.

I don't think that's gonna
happen, but no matter what,

we just have to
be really cautious

and keep a great eye
on Lisa's healing.
