My name is Heather.

My face hit a fence at 50 miles
per hour and it got crushed.

I was a pretty active kid.

Really outgoing.

I had tons of friends.

When I was 14, I was
doing a school project

with a classmate of mine.

She had some friends
of hers come over,

and they started riding
a motorcycle around,

and they wanted
to give me a ride.

And then,
all I remember is just

taking off super
fast down the road.

We ended up hitting the fence

that was at the dead
end of the road.

The next thing I remember was
waking up in the hospital.

I was so scared I was
gonna die that night.

My face just hurt so bad.

And I remember
looking in the mirror.

I didn't even recognize
who I was looking at.

I got split from
the tip of my nose,

all the way down to
the back of the throat.

Busted up my nose, broken bones.

Broke most of the teeth
in the front of my mouth.

Split the roof of my
mouth all the way down

to the back of my throat.

Crushed all the bones
in my cheek bone,

under my eye socket.

I had stitches

all over.

Initially it was 400
stitches inside and out.

I looked like a monster.

Over the next 11 years,
I've had over 10 surgeries

just to make
everything work again.

My sinuses had to
be rebuilt inside.

I had to get numerous
eyelid surgeries

just to have my eye function,

'cause I could never
close my eye all the way.

Even after all the
surgeries I've had,

I'm still left with not being
able to breath properly,

all these scars on my
face, and a crooked nose.

I went from this
fun, outgoing person,

to pretty much just
being a shut-in.

Always wanting to be indoors
and covered up all the time

because when people ask me
about the scars on my face,

it makes me have to relive that
moment again, over and over.

[birds chirping]

At home,
I really enjoy painting

because I can just block out
anything that's bothering me.

I've always dreamed about
selling my art in a gallery

or putting it on display.

But having all these
scars on my face,

I always feel like people would
probably be staring at them

and wonder how I've gotten them.

My name is Sonya, and I
went in for a mommy makeover

and came out a hot mess.

When I was little
I was outgoing,

the star, center of attention.

I wanted all eyes on me.

As a preteen with double D's
it got me lots of attention.

Probably the wrong attention,
but I liked my boobs.

Actually, I loved 'em.

At 16, I decided to go on a
hot date and got pregnant.

First time having sex.

Over the next 15 years
I had seven children.

Having so many kids,
my weight has fluctuated.

High, low, losing
weight, gaining weight.

After having my twins in 2016,

I reached my highest
weight of 307 pounds.

I felt like an Oompa Loompa.

I decided to have
a gastric sleeve.

I've lost 170 pounds total.

A whole person, a whole
plus-sized person.

After losing all that weight,
I had a lot of loose skin.

It looked like a mudslide.

I went to the doctor in Miami,

because he was a lot cheaper
than the California doctors.

I wanted skin removal,

but I was actually
sold a mommy makeover.

So, did you bring me here
because you wanna hike again?

Hopefully it won't
be like last time.

You seemed like you
were trying to avoid me.

I just feel like you're
ashamed of how I look,

so I kinda don't wanna
be next to you sometimes.

I'm not ashamed,
you're just mean.

Well, wouldn't you be mean?

I don't think so.

The doctor never asked
me what I wanted.

He basically said hi, lifted
my boobs, said we can do this.

And after waking up from
surgery I just felt it in my gut

that something was
wrong with my breast.

I went to the
restroom and I looked,

and I noticed my
right nipple, black.

I showed the doctor.

He brushed it off
like it was nothing,

and he basically said
put ice, massage it.

A few days later
I went in again.

This time, my boob was
inflamed and he said

you need to be admitted
into the emergency room.

After being in the
hospital for a few days,

the surgeon removed my
areola, nipple, and implant.

And when I woke up,

where the areola and
nipple were was gone.

I could see this meat, flesh.

Then everything started
changing on my stomach.

A drain fell out,
and then the openings

just started opening.

I felt like I was gonna
die in that hospital.

Feel like I have
a butt right here.

After three months of healing,

my body looks worse
than when I went in.

I don't wanna be seen like this.

I can imagine you
standing next to me.

I'm sure you don't wanna be
seen with me like this either.

I love you, but right now
you make me miserable.

I make myself miserable
with everything going on,

and I take it out on you.

When I got home I
pushed everyone away.

I didn't want to be seen.

I didn't wanna to be talked to.

I spend my days in bed.

I don't wake up to get
my kids ready for school.

about your story.

Can you enlighten us

and tell us exactly
what happened?

I know when you were young?

I was about 14 years old.

I ended up on the
back of a motorcycle,

riding on it my first time.

It was at night, and the
last thing I remember

is just taking off as fast as
you can and hitting the fence.

It split me from here

all the way down to
the back of my throat.

So, half my jaw
dropped in my mouth,

and it crushed my cheekbone

under my eye as
well, completely.

What kind of fence
did you run into?

Oh, a chain link fence.

Yup, yup.
Oh!

I was about a foot away
from a telephone pole.

I didn't think I was gonna ever

look the same again, you know?

'Cause I had stitches,
and I was bruised,

you know, on my face.

It's hard to talk about.
[laughs]

[Dr. Dubrow] Yeah.
I go back to that, you know?

You had how many
surgeries on your nose?

I'd say at least three.
At least three.

Nasal surgeries?
[Heather] Nasal surgeries.

It's so caved in on this side

that when I'm breathing
I can't even breathe.

[sniffs]
Normally.

We see a lot of patients that
have had trauma like this.

And when they have the scars,

and every time you
look it reminds you.

Causes a little bit of PTSD,

posttraumatic stress
disorder, sometimes.

What Heather went through
was extremely traumatic,

and it's something I'm sure
she doesn't want to relive.

Do you have any, a photo
album, or anything like that?

For us to look at.
I do.

I don't know.
It's actually

a photo box over there.
But I think there's
some pictures over here.

If we could do
something to help her,

that might help erase some
of these horrible memories

of what she had happened to her.

Okay, so your mouth is,

your lip is
completely split open?

Your teeth?
Everything, all the way up.

Looking at these pictures,
now it's starting to make sense.

Heather was a really young
girl when this happened,

and looks mean
everything at this time.

And she was traumatized.

Despite the good job
that the doctors did,

you can tell that
Heather's been through

many reconstructive surgeries
and still doesn't look good.

And that's one of the things

that she's really
self-conscious about,

so if we can do something
to help her there,

that'd be fantastic for Heather.

We have a pretty
good enough history.

Yeah.
[Heather] Okay.

And our next step is
really to examine you.

[Dr. Nassif] All right.

Let's do it.

We need to evaluate
Heather's nose, and her eye,

and the scars on her face so
we can see how can we help her.

Close your eyes tight.

[Heather] When the doctors were
asking me to pout

and move my face in all
these weird positions,

the only time I do that is
when silly play with the kids.

Let's look at this first.

These are all pieces
of tip cartilage.

[Dr. Dubrow]
Yeah.

Here, why don't you feel that?

Ooh, it's like
eggshells in there.

[Dr. Dubrow] When a patient
has had a very significant

traumatic injury to their nose,

sometimes the nasal
bones splay open

and are broken like an eggshell.

Look at this,
the medial bone here.

Let's pull 'em down.

Very significant, very.

So, one thing that
he's pointing out

is the inside corner of
your eye, on the lower.

It's pulling down.
It's pulling down.

So, you have dry eyes.

I do, I tear up.
You do?

You tear?
Mm-hm.

So that's not good.

Did they do anything with
the corner of your eye?

They took out skin from
under here, inside there.

They sliced that and
put it under here,

which made this tighter.

And now my eyes don't
close all the way.

So I always tear up.

Oh, your eyes don't
close all the way?

No, they don't.

So that's not good.

I know!
Fine.

I know.

If we play on this
side, and try to work

under bulk in this fat,

and there's a muscle
underneath there,

that muscle can get injured.

That can hurt your cornea.

At this point, I wanna say
let's leave your eye alone.

[Heather] I'm really
disappointed that the doctors

can't do anything about
the scars under my right eye.

I was really hoping to correct
the darkness under there

so it doesn't look like I'm
facing a black eye every day.

The cartilage in your nose has
been pretty much destroyed.

Mm-hm.

So, we have to be very careful.

If I try to
re-fracture your bones,

do anything like that,
there's a very high likelihood

that your bone will
just collapse in.

Even if everything
goes smoothly,

and you heal properly, and
I were to do this perfectly,

you still may not
like the result.

me that my nose is broken
in pieces,

it made me really nervous,
and I'm really hoping
that they're gonna be able

to put all this back together.

aesthetic aspect.

This is your fourth surgery.

I know. That's okay.
But you're gonna

have a better nose.

Heather's had her nasal bones

fractured in multiple
small pieces.

It does make me nervous
about how can I fix this.

What he's going to try
to do is, essentially,

put Humpty Dumpty
back together again.

And what I find ironic is that

he's kind of shaped

You know, I used to
wear jackets like that

with collars like
that back in the '70s.

Lemme tell ya that.

[Dr. Nassif] After examining
Heather, there really is
a lot of damage,

and this is a complex case.

I hope after fixing her
nose that this can help

ease some of the
emotional trauma

that Heather is going through.

[Dr. Dubrow] Have you ever
seen a picture of yourself

doing an activity many years ago

that you thought at
the time was cool

and then, in retrospect,
now you think, oh my God?

You're almost
embarrassed about it.

Well, what did you do?

No, you know this Ferrari
that you drive now?

Haven't you ever seen
a guy in a Ferrari,

you look over and it's
always an old dude?

So what are you saying?

You're saying I look
too old in my Ferrari?

I look 42.

Okay, but the point is--
A 42 year old looking guy

can't drive a Ferrari?

I'll admit it, Paul looks 42.

A really [bleep] old 42.

Our next patient's
name is Sonya.

Sonya lost a bunch of weight,

and then had the usual.
[Dr. Nassif] Oh, jeepers.

Lax skin, both in the
chest and abdominal area.

I know, I don't know
what that skin is.

And the abdominal contour is

[Dr. Nassif] Like a big ledge.
[Dr. Dubrow] All right.

Can you please send in Sonya?

Hi.

Terry Dubrow, how are ya?

So, Sonya.

Yes.

How did you get to this point?

I gained a lot of weight
throughout my pregnancies.

I had seven kids.

You have seven kids?
[Sonya] Mm-hm.

[Dr. Dubrow]
How old are you right now?

34.
Wow.

You had surgery in Florida, but
you live in California, yes?

Why did you have
surgery in Florida?

Because it costs an
arm and a leg here.

Plastic surgeon?

He said.

So you didn't check
the credentials at all,
on the doctor?

Uh-uh.
He's not board-certified.

You should always be able
to answer the question

as to whether your doctor
is board certified or not,

because that's
indicative of the fact

that you're doing
proper research

before selecting your doctor,

because plastic surgery,
at the end of the day,

is real surgery and often
it can be real dangerous.

So he's probably not
even a plastic surgeon.

Probably was a general surgeon.

And then what happened?

I came out of surgery,
and I opened my bra,

and one of my nipples was black.

So he immediately bumped
up the blood supply?

And what'd they do?
He brushed it off.

It was normal, put ice on it.

Put ice on it?!

[Sonya] Sure did.
Let's make it worse.

Put ice on it.

Putting ice on a nipple
that's turning black

is the last thing you wanna do.

You wanna increase
the blood flow,

but putting ice on it is gonna
constrict the blood flow,

making it harder for
oxygen to go through,

making it more difficult for
the nipple to stay alive.

Putting ice on a black nipple

is neither diagnosing
nor treating.

We call that, in plastic
surgery, malpractice.

I came back a few days later,

and he said you gotta
go admit yourself

into the emergency room.

What, when you were
red and infected?

Huge.
[Dr. Dubrow] The right breast?

The right breast.
[Dr. Dubrow] Yeah.

I was there for a month.

Jeepers.

[Dr. Dubrow] Okay, and how
did the tummy look?

Like I came out with a butt
on my stomach, no joke.

Okay, boy, this must have been

a strain on your
relationship, huh?

Well, I turned into a
royal pain in the ass.

You did?
Did she?

[Sonya] I did.

So, I'm gonna ask
you a question.

Her right breast,
she has her nipple,

and she has a very
uneven tummy tuck.

Does that make you
not wanna touch her?

It's not as attractive, no.

When she was heavy
she was happy.

And maybe 'cause she's
unhappy with herself,

she takes it out on me.

Is that your perspective?

Yeah, I look a mess.

[Dr. Dubrow] Do you
take it out on him?

Are you gonna support her

if she has another surgery?
Of course.

I preferred Sonya
when I first met her,

when she was heavier.

Particularly 'cause
she was happier.

I'm in love with her.
I care about her.

I'm hoping this'll bring
her back to being happy.

By the way, 99% of the people

sitting in that chair right now

would say it doesn't bother me.

I love her for her.

You're maybe the first
one in a long, long time

who's actually being
honest about that.

are a tough case.

So, let's go examine you.

The Sonya Tom fell in
love with was outgoing,

was bubbly, was
fun, was playful.

We had so much fun together.

But losing all this
weight and ending up

with a deformed body, I'm
not a nice person right now.

Yeah, he doesn't deserve that.

He's an awesome man.

So let's look at the
chest first, okay?

Let's put your hair back.

So you have a breast implant
in place that's soft.

This one is a problem child.

You only have a
fraction of your areola.

Probably the best thing to
do is remove the skin graft,

and try to put a big
implant in there,

and expand up your
regular breast skin.

'Cause the skin graft is
necessarily so usable.

And then when it comes to
your tummy, do me a favor.

Stand up for a second.

The problem with
the abdomen is that

she's got this pubic area
that's contracted upwards.

So it's pulled up the height of

your belly button, right?
[Sonya] Yeah.

And your belly button is gone.

It's just a basically a scar.

I mean, this looks
like work that was done

by a surgeon who read a couple

of plastic surgery textbooks,
to be honest. You know?
[Sonya laughs]

[Dr. Nassif] Terry does
a lot of difficult cases.

But looking at Sonya's
findings,

I've no clue how he's gonna
be able to repair this.

This is tricky.

Okay, so here's
the thing, Sonya.

Even if everything goes
smoothly, and you heal properly,

and I were to do this perfectly,

you still may not
like the result.

[Sonya] I hope the surgery's
gonna help me

be a good person that I was

prior to this nightmare
that I've been through.

Well, I worked
hard to lose weight

to have the body that I
wanted, to be healthy.

And I saved my money.

I have seven kids, it
wasn't easy to save money.

You're mad at the universe

for this complication.
I'm mad at me.

I feel selfish that
I used that money

on myself and this happened.

So, this is my
feeling on this, okay?

I think we can go ahead
and do this together.

Sonya's been through a lot.

She had a gastric sleeve,

lost a ton of weight.
Mm-hm.

And then had a mommy makeover.

[Sonya] I did.

But it was like a
Frankenmommy makeover.

Right?

Sonya worked really
hard to get her body

where she wanted it to be
after 160 lb weight loss.

What I'm hoping is that
I can be the final step

to make that major
transformation that
that makes her feel good

and is in line with how
much hard work she's put in.

[Dr. Dubrow] I'll begin
Sonya's surgery today

by removing the skin
graft on her right breast,

then manipulate the remaining
tissue of the breast envelope,

insert a new implant to
give her a nice shape

while also performing a lift.

For the left breast, I will
perform an areola reduction,

then replace her current
implant

with a smaller size to
give her breasts symmetry.

Then I'll move on to her tummy,

and making an incision at
the bottom of her abdomen,

I'll create a new belly button,
tighten her abdominal wall,

and remove excess skin to
achieve a more natural contour.

Okay, let's remove
the skin graft.

It's a patch, it
served its purpose,

it allowed her to
close her wound,

but this thing has gotta go.

Look at this, that gigantic
wound on her thigh,

that donor site could get
a skin graft this big.

Yeah.
It's a long way to go

for this very short slide.

So, now I'm gonna go in

and see what the old

a new breast pocket
that wasn't subjected

to the complications
she suffered previously,

and will potentially
increase the chances

that she'll heal
without any problem.

gonna use a 310 CC

ultra high profile implant.

And this is the side we will
try to match the left side to.

So the good news
is we have a good,

really good soft tissue
pad overlying this implant.

Okay, let's go off
to the left side.

Now, let's go in the pocket.

Here's the implant.

Implant is a, wow.

Big one.
Big one, 650 CC implant.

That's a big implant
to put in someone

with a lot of breast tissue.

So, that put a lot of pressure

on the underlying nipple
on the other side.

Plus, we had a pocket that goes

all the way up into her armpit.

This guy opened up
this entire pit area.

Look at that.

All the way up there.

This guy who did this
operation was in a hurry.

It was discount plastic
surgery at its worst.

I can't believe the size of
Sonya's breast implant pocket.

That's indicative of a surgeon
who clearly was in a hurry,

and just opened up everything
and threw an implant in there

to try to move on to
the next procedure.

Okay, we've got good symmetry

between the right
and left breast.

We got the left pocket
tightened very nicely

around a new, smaller implant.

Okay, that's good.

Oh, I don't even
wanna think about

what we have in store for
us here in the abdomen.

If you thought the breast
was hard, just wait for this.

After seeing the level

of what we call technical
misadventure in Sonya's breasts,

I'm actually a little afraid
to go down to her abdomen.

I have no idea what
I'm gonna find there.

The challenge here is to
try to release everything

to get the pubic area
back to where it was,

'cause he pulled it very high.

Look at that, it's been pulled
up for so long that now,

when you release it,

it's this strange,
lax, wrinkled skin.

This case just got
significantly worse.

So, we flexed her.

Now we're trying to make
the skin nice and tight

without pulling the
pubic skin up too much.

My challenge with lowering
Sonya's pubic incision is that,

as I lower the pubic skin,
I'm making it more lax,

essentially, trading
one problem for another.

Okay, so we've
restored the pubic area

to its normal anatomy.

That's good.

We've removed all the old, icky,

soft tissue that
basically scarred down.

Then we also have to
make a belly button,

which is not an
easy thing to do.

That will be a belly button.

We've basically given
her a normal abdomen.

Mm-hm.
Okay.

I'm really happy with this.

The breasts are symmetrical.

They're a nice size.

They have a very good shape.

The abdominal wall
is completely normal.

It looks like a
regular tummy tuck.

There's no central scar.

Her pubic area's completely,
normally, totally restored.

about the medial part.

So, guess what we're
gonna do today?

on your right eye.

So, Dr. Massry, a buddy
of mine, colleague,

you know, he's gonna

Today is Heather's
fourth rhinoplasty.

She's had multiple
facial procedures.

You know, I really hope this
is Heather's last surgery,

because we wanna put this
motorcycle incident behind her.

Heather will be having
multiple surgeries today.

My team will begin by
harvesting rib from her chest

while I open up her nose.

I'll perform a turbinoplasty
for better airflow,

and we will then lower her tip
and open up the nostrils
with composite grafts.

Moving on to her right eye,
we'll start by making an
incision inside the eyelids,

and identify the grafted fat,

and dissect it from the
muscle or natural tissue.

Next, we'll middle
the corner of the eyelids

by performing a tarsorrhaphy,

partially sewing the upper
and lower lids together

to help lift the eye
and give her more symmetry.

We're making our incision
for our rhinoplasty.

Already, I can tell right now,
there's already a
lot of scar tissue.

Skin is thin, yeah.
See, that's what's
scary about this,

'cause of her multiple
surgeries, you know,

just one little too superficial
snip and then were too low.

Heather's skin
is extremely thin
from her previous surgeries,

and that means we have
to really be careful

when we're elevating the
flap, because one wrong move

and my scissors pokes
right through her skin,
and that will create a scar.

And now, we're gonna dissect
the rest of the nose,

and then start
the reconstruction.

Oh, those are huge
phenodes, huge, bony spur

impinging the hole,
left airway.

Okay, it's really important
we get these spreader grafts

absolutely perfect,
'cause this is the key
to making her nose straight.

By just elevating the spreader
grafts a little bit more,

we'll have a better
aesthetic dorsal line

right down the straight
part of the nose.

Okay, her airway's gonna
be a lot better, look.

We opened up the whole nostril.

Dr. Massry and I have been
talking about Heather's eyes,

and we formulized a plan.

So, she got a vertical
cut right through

her lower eyelid margin.

Just think, if that woulda
went a little bit deeper,

boom, the eye is gone.
She's blind.

That fullness is grafted fat.

The implantation of dermis
fat graft, or via injected.

That is a huge fat pad.

When you're de-bulking
an eyelid area

that has had fat transplants,
it's very hard to judge this.

So, you gotta take out a
little bit, take out a lot,

then you have to
reevaluate the area,

and make sure you
actually don't overdo it.

Guy, you did a great
job on that eye.

Pretty much matches
the left side.

Awesome, now it's
really up to Heather.

She's gotta heal well.

The chance this belly
button will make it

is severely reduced, because
it isn't a real belly button.

surgical trauma

caused so much of an

and I also took a piece
of shaved cartilage,

and we put that also

to make it more symmetric.
