Because back in my heyday,

We've been together since high school

and I certainly don't remember any lines

forming around the corner for you.

Now how I got that
recipe on the other hand

is another story.

- Oh, yeah?
- Yeah.

But what I can say is that Mrs. Hart

told me to guard it with my life.

- You do that.

- I miss her.

You know, it seems like just
yesterday that she passed.

I just have to ask myself

if we could have done more to
address her cancer earlier on.

You know, since I've started
this MD residency program

I have entirely different perspective

on cancer services, treatment,
and preventative care.

With every cancer patient
I come across who's beat it

I just can't help wondering if, you know,

we could have done more.

- Done more?

Like what?

- Like I don't know, you know,

maybe if we had pre-screened earlier

or maybe if we had started treatment

right after she was diagnosed,

or, you know, if she would've
just listened to your dad

at the beginning, maybe more
could have been different.

- Like how?

More pain?

More suffering?

Experimentation?

Different how?

- I didn't mean to mean to say that.

- You didn't need to say that?

So you just, you didn't mean
to show how naive you are?

That's what you mean?

You didn't mean to show
how blinded you are

by your own ambition?

Doctors, medicine, all of it.

It doesn't help.

It just speeds up the process of death.

Chemo just made everything for her worse

and my father's advice
just fueled the fire.

- No, that's not the case

and chemo wasn't the only option.

- It was like she was there.

And the next day she's
being sentenced to death

by the very doctor she went to for help.

Had she not listened to my father's advice

and just ignored him completely,

she would have went out on
her own terms in her own time.

But nah, she suffered through hell to die.

Talking about 5% survival rate.

What the hell does that mean?

Why do they tell you that?

Because false hope sells.

She had no chance, no
chance of beating cancer.

Even if she was predisposed genetically,

like they say I am, but look at me.

I'm fine.
- Junior.

- No, the bottom line is this.

My father poisoned her mind

and the doctors experimented on her

like she was some lab rat

or a guinea pig, and
guess who lost in the end.

She did.

All of those tests, man, the injections,

the medication, the chemo, all of it

that he forced down her throat for what?

So he could, so he could
make up for lost time?

So he could make up for never being there,

never giving her what she really wanted,

which was a husband.

Stress is what killed her,

and the cancer was secondary.

And the doctors, they just
put the nail in the coffin.

- The reality is that most
families have a stigma

against cancer treatment.

And you know, your dad was the exception.

He was!

He encouraged your mother
to go get treatment sooner.

But by the time she took him
seriously, it was too late.

- Too late for what?

She should have never had to go through

what she went through because
of her selfish husband

relying on false hope and optimism.

He kept her connected to a machine

for two weeks after she was declared dead

hoping for a miracle.

There are no miracles.

(somber music)

And if it's me, if I am
ever pronounced dead,

or slip into a coma with unbeatable odds,

pull the damn plug.

Don't let that man, the poster
boy of false hope and faith

convince you otherwise.

Pull the plug.

Promise me because that's all I'm asking.

- I'm so sorry, Mr. Hart.

For inviting me over

It's been a while since I've
seen you all, my loving son.

about the bun you have

cooking in the oven,

you know, the one that mom used to make,

I know you blame me for your mother,

and the medical recommendations

I encouraged her to pursue.

Some she did, some she did not end that.

- And I know you haven't forgiven me

for some of the poor choices
that I made in the past.

It's true, I could have
been a better father.

I'm manning up.

And even a better husband.

But that doesn't discount the choices

I encouraged your mother to take

when she was sick.

Most, if not all she refused to do

out of fear that it would
just make things worse.

- And it did.

- However, the advice that she did heed

definitely was in her best
interest, prolonged her life,

and had she taken them sooner

there could have been a
positive outcome, and your wife

and aspiring medical
doctor can attest to that.

- Leave her out of it.

You know what your
recommendations got my mom?

Just more pain and suffering.

- Junior, now, you listen.

- No, I'm not listening to you anymore.

No one should.

- Junior, we could have worked together

as a team when your mother was sick.

And if you had faith in
something greater than you and I

even the size of a mustard seed, Junior,

then who knows what
mountains we could have moved

in regards to your mother's treatment.

But since you're so God damn stubborn

and always wanting to blame
somebody, blame yourself.

God damn it.

- Yeah?

for not working together and
not having a positive attitude

when your mother needed it the most.

Black Friday only became

a recent phenomenon, let's say
in the last 10 or 15 years.

- I can never understand Black Friday.

I don't know why people
would stand in a line

for hours for a bargain, but.

- Yeah, but they can't form
a line at a nearby clinic

or at a doctor's office for
a routine medical checkup.

- Here, honey, you should eat something.

- Actually I lost my appetite.

- Junior, stay.

You know your father is a fighter.

He's not the only one fighting.

It's these and the headaches he's having

that I'm more concerned about.

- He's been self-medicating with those

for as long as I can remember.

- And you haven't done
anything to deter it?

And you haven't encouraged
him to seek medical attention?

I mean, you're an aspiring
medical doctor, are you not?

Well, aren't you?

- We all have choices in how
we choose to live our life,

including your son, my husband, Junior.

- And what if the person simply is blind

to what the best choice is like
my son, your husband Junior?

- What am I supposed to do?

He's made his choice.

That's his choice and his alone to make.

- Well, if that's the case then

regret is the by-product of inaction.

Let that settle with you.

And hopefully, your choice
of inaction won't give birth

to a lifelong regret.

It was my choice not to encourage my wife

to seek medical treatment

when she first saw physical
changes in her body.

Instead, I did nothing,
stood there and watched

while she waited until
the absolute last minute

to seek treatment when
there was no other choice.

And by that time it was too late.

Junior hates me for
trying to save his mother.

And I only came to her
aid at the last minute.

So no matter how you spin
it, it's a lose-lose for me.

I've lost my wife, and you
might as well say I lost a son.

God forbid there's a more serious issue

underneath the surface of those
migraines Junior's having,

and hopefully the daughter
that you're soon going to have

won't hold you accountable
for your choice of inaction.

but not before I have
some of this tasty bun

and a plate of this delicious food to go.

but what I can't understand
is why do black lives

only matter when a cop kills them,

but not when they kill each other?

- You would think that
because you're black

you would understand
the life of a black man.

When black people kill each
other in these communities

it's a failure at an institutional level.

- Great, so blame it on society, typical.

But when if ever does anyone march

against black on black crime?

Exactly.

Hardly ever.

- There are plenty of
good people who march

against gun violence in these communities,

myself included, and it's like

it's the reason I police the way I do.

All I'm saying is if you
look at these communities,

there's drugs, guns, and
there's no opportunity.

I'm just, they're basically
set up for failure.

- I'm on the phone with
his wife right now.

She's on her way into the building.

- I don't know, Mr Hart, I just got here.

(dramatic music)

(indistinct shouting)

I'll let you know the
moment I find out anything.

Okay, I'm with his partner now.

- He's in an emergency MRI unit.

Just head that way

and I'll send Mr. Hart
up when he gets here.

- All right, no problem.

- Mrs. Hart?

- Hi, yes.

- I'm Dr. O., Chief Neurologist here.

- Where is he?

- He's undergoing an emergency MRI.

Come with me.

(dramatic music)

Ma'am, your husband's in a coma.

Has he had any headaches lately?

He has four large to mid-sized tumors

in different areas of his brain.

We used a combination of infrared lighting

and protein biomarkers
and there appears to be

a significant presence of glioma serum,

which would indicate that the tumors

are not only inoperable
but cancerous as well.

- Got here as fast I could

I'm his father, Daniel Hart, Senior.

Is he gonna be okay, doctor?

- He's in a coma.

- With four inoperable tumors
that I suspect are cancerous

Is there a family history of
cancer that you're aware of?

- My wife passed from it
but what I don't understand

is the relationship
between my wife's cancer

and the coma that my son's in now.

How's it all related, doctor?

- The cell mutation appears to be genetic.

- From what we can tell
so far, the largest tumor

is in the medulla region
of the brain here.

The other ones which are
significantly smaller in size

are located here, here, and here.

- We have less than one hour
before the largest tumor

fully invades this area
here, which once it does

will leave Daniel brain dead.

- Unless of course you decide

to keep life-sustaining measures in place.

Physically he's in a coma

but beneath the surface on a
subliminal conscious level,

he's awake almost in a dream

fighting for his life from the inside out.

- What are his choices?

- There's less than a one percent chance

he'll survive if we attempt surgery.

Chemotherapeutic drugs or
chemotherapy will likely fail.

It would be virtually
impossible to get the drugs

to the precise area of the brain

where the tumors have already invaded.

I'm truly sorry.

There's really nothing else
we can do at this point.

Life-sustaining measures would
offer some level of comfort

until the tumors take over
and his functions cease.

- He wouldn't have wanted that

to be kept alive by a machine.

If there really are no other options

we have to pull the plug.

- There must be another option, doctor,

but my daughter-in-law will not decide

what becomes of my son
without us exploring

whatever else there is.

He's my son, God damn it!

(dramatic music)

- It's happening.

The tumors, the dark
cells, have infiltrated

this part of the brain, the safe zone.

- He would not have wanted this, doctor.

- After having to bury a wife,
I refuse to bury my only son.

There has to be something else.

- His vitals are all over the place.

- Yes.

- Junior is your?

these circumstances.

I know a lot's happened
in the last couple hours.

I just reviewed the scans on on your son,

on your husband, and I
overheard what you were saying

as I walked in and I
would agree with that.

He has a brain tumor

and it looks like there
may be four parts to it.

So this is a serious situation.

And uh, fine that's good.

Okay, thanks very much.

- Thank you.

- Carol?

- These are very powerful
and aggressive tumors

Think of them as home invaders.

- Invaders.

- Coma is the body's natural defense.

These tumors have begun a full invasion

of the area of Daniel's brain
that control his senses,

the parietal lobe.

- Who is that?

- How do we know if he's
even still alive in there?

- There's no way for us
to be sure at this point

but it'll depend on whether
his body's natural defenses

are able to overcome the tumors
that have already invaded

this area of the brain.

- So then what's the point if
he'll just end up brain dead

or left hooked up to a
machine until his heart stops?

He would have never made such a choice.

- Nor can you for him.

- I will make whatever choice is necessary

so that his survival is
not dependent on a machine.

- What makes you so sure you
made the right choice, Carol?

You had no courage to
make the right choice

in a matter of life.

So what do you think you
can do in a matter of death?

- What's happening, doctor?

What's wrong?

- This tumor here is
beginning to overpower

his body's natural defenses.

Think of that area of his brain

as an area of safety and security.

In other words, his home, safe zone.

He's the body's natural defenses

and the tumors, or dark
cells, are the invaders.

If he's unable to overcome on his own

will bring him deeper
and deeper into a coma.

- And then what?

- Death.

- Don't you dare give
the order, have faith.

When my wife passed away

I heard word of another
medical treatment, nano.

- Nanos.

Yeah, biodegradable nanoparticle vectors

capable of selectively
targeting brain tumors.

For it to work, the nanos
would first need to navigate

the complex architecture of Daniel's brain

then it can selectively
distinguish between cancer cells

and healthy cells before
delivering a neurotoxin.

It won't work.

- Why not?

- Nanos can't be delivered
intravenously, through a vein.

They don't cross the blood-brain barrier.

Infusion is unpredictable,
they easily can miss the mark.

Convection is a possibility,

but we would need to slowly insert

at least three nanoparticles
here in the safe zone

marked with an iron oxide marker

so we could track the progress
with the MRI adaptive imager.

Imagine a team of Navy Seal divers

emerging from an ice covered
ocean on an extraction mission

in a bid to help Junior survive

these powerful and deadly tumors.

This approach is highly speculative

and only makes sense if
there were some indication

that Junior hadn't already given up.

- Light shines in the darkness

and darkness has to not overcome it.

So where there's faith,
doctor, there's always light.

- Do it.

- Prep the patient for neurosurgery now.

Okay, we're gonna do the first incision

and make sure that everything
looks good on the monitors.

What the hell is happening?

Who hell is trying?

- Friendly, whoa, whoa, whoa!

- We're here to extract
you to the safe zone, okay?

- We'll protect you along the way.

- We got a lot more coming!

- We're also here to identify, track,

and eliminate these dark cells.

(gun shots)

- Without our help, these
cells will kill you.

- The injection was a success.

We put the nanoparticles at the tumor site

with the most brain activity here,

but before we can claim victory

the nanos must first get here.

Along the way they have
to find and eliminate

all the tumors they find.

First they must eliminate
the one here in the safe zone

then they have to track, locate,

and eliminate mid-sized
tumors here in the gray area

which if I had to compare to anything

would be to a densely wooded forest

controlled by a deadly predator.

The last tumor and the
largest tumor is here

in the medulla, a region of the brain

that if I had to compare to anything

it would be his solace or
church, spiritual core.

This tumor.

This tumor is unlike the others.

This one feeds off the medulla.

Now the nanoparticles level
the playing field somewhat

but absent a miracle they don't
completely even the score.

- Somebody needs to tell me
what the hell is going on here.

everything that's in there.

It appears the body's natural defenses

are reacting negatively
to the nanoparticles.

and eliminate them while
they're still weak.

here in the gray matter.

No man's land.

Where the larger tumor
exists that we're after

and we need to extract
you to your medulla,

your spiritual core, your church,

where there's a larger tumor

and the last tumor to
exist that has invaded.

Okay.
- Get him out.

Let's go!

Number three, pop the
smoke and lay cover fire.

- I got you covered.

- Number two, one, go!

(intense music)

- One of the tumors in Daniel's safe zone

has been significantly reduced in size

but there's another one.

One of the nanoparticles

has isolated the other
tumor in Daniel's safe zone.

- Faith in things unseen.

- Where's three?

(explosion)

- We lost one of the nanoparticles.

I don't know what went wrong

but whatever it was
generated a massive shockwave

of synoptic energy in the
safe zone, the frontal lobe.

The good news is that whatever it was

completely eliminated the tumor
in that area of the brain.

There's still two nanoparticles left

which will support the
body's natural defenses

but still there's no certainty
as to whether any of this

is going to pull him out of a coma.

- See those woods over there.

If you thought those
dark cells were tough,

wait until you see on that side.

- I don't get this.

If this is not a dream

and it's not a nightmare,
what the hell is this?

What aren't you getting?

This isn't a game anymore.

Your unreasonable way of thinking
