we like to see a distance

of good news.

When you first look
at jessica's breasts,

you think that her nipples
are very high

when in fact her nipples
are in an okay position.

It just looks that way

because the pockets
are so low.

Your breast envelope
has been stretched.

So your breast implant's
this big,

but your pocket is this big.

One of the harder things
to fix

in someone who's lost
a lot of weight

is that.

When you lose a lot of weight,

you forever lose
some of the skin integrity,

and even if you tighten it,
it can loosen up.

Let's go to your abdomen.
Stand up for a second.

This had been lifted
and sucked.

You have laxity
of the pubic area.

You have contour
irregularities here.

You have pigmentation issues
from the scars.

Even if I cut them out,
there's still gonna be a scar

in an abnormal place
on your abdomen.

I can make an incision,

tighten your abdominal wall
to give you a better contour,

pull as much as I can down
to remove scarred-up skin,

but the whole thing
can die on me

because it's been so badly
abused and beaten up, you know?

Jessica's surgery is one

of the most extraordinarily
complicated procedures to do

because the skin has been
so stretched out,

it has a lot of internal
and external scarring,

and the ability to get
a really good result

After my first septoplasty,

my nose did not look right

It was crooked. The tip
was going to the right.

Had a rhinoplasty done

about a year
after the incident

to fix the tip and kind of
reshape the tip of my nose.

Different surgeon
or same surgeon?

- Different surgeon.

This doctor sounded great.

He was an ent
and a plastic surgeon.

When it was all said and done,

just came out looking
like not good at all.

What's happening now with you?

The appearance isn't, you know,

where I want it to be,
obviously.

It collapses when I breathe
deep breaths in.

I'm having snoring issues.

obviously, at night.

Yes. Some days are
better than others.

It's 4:50 a.M.

in chemotherapy,

and it led to a dependency

which eventually led

a rehabilitation program

to get off of it.

I'm off of, uh, oxycodone,

and now they've moved me
onto a maintenance therapy

where I take, uh, a methadone.

It's been
almost three years now,

and I'm ready to start weaning
off of that program as well.

Yeah, I mean, you know,
it's been a good amount of time,

especially since
you don't have chronic pain.

You should be
weaning off that.

Yeah, without a doubt.

It just bothers
the crap out of me

that doctors do this.

The opioid crisis
in our country is an epidemic.

We need to monitor doctors
that are giving out narcotics.

It's killing a lot of people,
getting people addicted,

or more of a gateway drug
to harder drugs.

I mean, this can kill you.

Whether I do surgery or not,

- you need to get off.
- Yes.

Looking back at it now,
if I had known then

what I know now
about oxycodone,

I never would have even
taken it.

and then insert
a new set of implants.

Then I'll perform
a breast lift,

removing excess skin
to tighten the breasts.

Moving on to the stomach,

I'll do scar revisions

to reduce the size
of her previous scars,

and then I'll make an incision

horizontally
across her lower abdomen

to remove excess skin
to tighten the stomach

and ultimately give her
an overall better shape.

fixing the very severe
deviated septum

and open up his airway.

For eric's surgery,
I'll open up the nose

while my team harvests rib.

Next I'll take down
the hump on the bridge

and perform a septoplasty

to repair his deviated septum
and improve his breathing.

Next I'll use
a supertip graft,

infratip graft,

and alar batten grafts

to fix the pinching
of the nose.

Finally I'll inject
some finely diced cartilage

along the dorsum

to fill in any imperfections.

All right, first step,
we've opened up the nose.

The cartilage
around eric's tip,

it's just collapsing in.

- See how weak that is?
- Yeah.

So when he breathes in,
it goes like this--

Part 2 is gonna be
taking down that little hump

that eric has.

Let's see how tricky

eric's deviated septum is.

That looks very complex.

The reason why eric's nose
is still crooked

is because his bones are
crooked and asymmetric,

his middle valve is crooked,
the cartilage here.

One side's wider
than the other.

The septum is still
very deviated,

and his tip is deviated.

He's got the mother
of all mothers.

this septum is complex.

I exposed the septum.

And it's pushing
all the way over

into eric's left nostril.

The best way for me
to fix that

is to use spreader grafts
to strengthen it

and also
a septal batten graft.

That will give us
a better airway

and a straighter septum
for eric.

In addition, to make eric's
lower part of his nose

and his tip good,

I'm gonna use a supertip graft

and an infratip graft.

That will not help
his breathing at all,

but his nose will look
a lot better.

Just a little bit
of finely diced cartilage

just going along the dorsum,

especially in the region
of the side of the tip

where it was collapsing
before.

That will just even out
the two nostrils

a little bit more.

Okay, looks great.

Eric is gonna be
the best-looking guy

at his wedding.

It's only been a few months
since the surgery,

but we've already noticed
a huge change.

He's much happier now,
and he's confident.

I've been on methadone
for almost three years,

and dr. Nassif really
gave me the push I needed

to get off my methadone.

Before my surgery,

I had two rhinoplasties done

by doctors who did
more trauma to my nose

than the fist did to my face.

But thanks to dr. Nassif,

my nose is supported,
it has some shape,

and it no longer collapses
on the right side

when I breathe in.

Now, when I walk
down the aisle,

I know that everybody will
be staring at my fiancée

and not my crooked nose.

after her check-in,

but unfortunately,

she had complication
after complication

and has required
multiple revisions.

So, you know, she got
a breast implant infection,

we have a lack of attachment

in the inframammary crease

which is making
the areolas and nipple

With time,

the implants have been weighing
on that inframammary crease.

It disrupts it, and you get
more of the bottoming out.

Sometimes the patients' bodies
just don't cooperate,

and that's clearly
the situation with jessica.

I never run out of patience,

literally, figuratively.
