at our top dozen patients

that traveled abroad

and as soon as I woke up,

I had dead skin.

jumped on a plane,
and went to Iran.

This is right up there as one
of the craziest stories

we've ever heard.

These patients of ours
weren't traveling

for a trip of a lifetime.

I just started vomiting blood.

He's not a plastic surgeon.
He's actually a gynecologist.

As we listen
to their foreign adventures...

‐ I went in coma.
‐ You went into a coma?

Did they put breast implants
in your buttock?

It was clear
those surgical souvenirs

were horror stories
they'd rather forget.

They both came in with, like,
a really big block of ice.

Dr. Ice was
your anesthesiologist.

Dr. Ice
was my anesthesiologist.

‐ You're kidding me.
‐ I'm not kidding.

‐ My implant fell out.
‐ Your implant just fell out?

They just stuffed a bunch
of random silicone

inside of my chest.

[ voice breaking ]
I felt like I was going to die.

Other countries don't have
the same patient standards

that the U. S. has,
and back on home soil,

these patients were desperate
for Paul and I

to take on their cases.

The big reveal was when
they took the cast off.

You're telling me
the plastic surgeon

operated on your nose

while you were under anesthesia?

pretty wild surgical stories,

and this first patient
is no exception.

Number 12 went abroad
for breast implants

and had one of the craziest
stories we've ever heard.

‐ Nice to meet you.
‐ We've had plenty of patients

come to us for exposed implants,
but I've never heard of implants

just falling
out of someone's chest.

I wanna get right into it.

Your complication is basically
from the Dominican Republic

by way of Columbia.

My name is Judy,
and I got done dirty

in the Dominican Republic,

and now I'm left
with bottomless boobs.

I first went to Columbia
to have a breast augmentation

because I was done
having children.

‐ What are you, 35?
‐ I'm 37.

‐ I'm about to be a grandmother.
‐ A young grandmother, huh?

Probably the youngest one
I've ever heard of.

So go ahead. What happened?

I then decided to have
my implants changed

to smaller sizes.

So then I went
to Dominican Republic.

It was a terrible experience.

I started getting wounds
on my incisions.

So the cells are dying.

I came back
to the United States,

immediately went
to the hospital.

She was checking it,
and she was, like,

going like this and feeling it,

and then pop, one fell out
right in her hand, and she's‐‐

What do you mean, one fell out?
One‐‐One what fell out?

My implant fell out.

‐ Your implant just fell out?
‐ The hole was that big?

It was a few holes. She's like,
"I've never seen this before."

‐ Neither have I.
‐ Yeah, I don't think any of us

have ever‐‐I mean...
‐ [ laughs ]

So, um, I went home.
They scheduled me for a surgery.

That was the right breast
that just poked up?

‐ That was the right, Mm‐hmm.
‐ What happened with the left?

The day of surgery,
I was brushing my teeth,

and I felt a pop.

‐ Pop out. Oh.
‐ Mm‐hmm.

The implant fell out
of your stomach or in the sink?

It was in my shirt.

They told me that I would need
to have wound vacs in,

and I would have to stay
in the hospital for two weeks.

The wound vac was, like, killing
whatever was inside.

It was so painful.

This tissue is not the most
friendly to a breast implant.

Obviously, I would much rather
not use implants.

What does your gut tell you
that you might choose?

Whatever you feel is best.

I just wanna look normal,
and I wanna be a GILF.

A GILF. Do you know what
a GILF is?

[ laughs ]

‐ I know what‐‐Yes.
‐ Wow.

GILF is a grandma
I'd like to [bleep].

Okay, we'll know
in a few seconds whether or not

this tissue
can keep things alive.

Yeah, there's some bleeding,

but I think we can probably

I need to release this tissue

to be able to fold
the sides together.

Do I have enough breast fullness
and symmetry,

and will she be happy
with that result?

I mean, it's kind of

Kendall Jenner type vibe,

So I'm pretty pleased
with the shape that I have

and doing any more surgery
to put in breast implants

takes her to a risk place
that she doesn't need to go.

Before my surgery,

I was both physically
and emotionally scarred.

But now, thanks to Dr. Dubrow,
my scars are gone,

and now I have two full, perky,
implant‐free breasts.

patient ordered plastic surgery

like Paul orders fast food.

I'm Carmen,
and I am 33 years old,

and I ordered my nose
off of a value menu.

Carmen's nose was so pinched,

it gave her a very significant
breathing issue.

What I didn't know is
if Paul could fix her nose

without making it look
significantly bigger.

I went to Mexico‐‐Guadalajara.

I was trying to save
a, you know, a coin, you know?

It was like going
to a fast food restaurant,

and you going on the menu board
and picking what you want.

They gave you a list.
You get three things.

You get one for free.

I said I wanted my nose.
I wanted a tighter face.

I wanted bigger boobs.

And I got my castration
for free.

I was born as Kevon Scott,
as a male.

I know you can't tell,
but I was.

When I got home from Mexico,
I had what I wanted

everywhere else
except for my nose.

Hamburger, fries, nose job.

You should not be ordering
a surgery off a menu.

You know to get up,
and you need to run out

as fast as your legs
will carry you.

I instantly found
another doctor in Atlanta,

and I got another surgery
after Mexico.

Actually, more than one
after Mexico.

And now I have to stick a straw
in my nose at nighttime

just to even get any air
in and out.

The doctor took
a lot of extra tissue

on the right side...
‐ Mm‐hmm.

...as compared to the left side.
I mean, really overdid it.

Can I go find it
and get it back in there?

It's probably a little bit too
late now to get that back.

Oh, okay.

Everything has been overcut,

overaggressively excised,

and also her nostrils have been
taken in too much.

I'm for it as long as
it's the last time.

I'm kinda nervous,
but I want this to be

my final and last nose job.

Okay, let's start with the nose.

Making my incision.

It's actually
a pretty nice deviated septum

of the left airway.

Wow. Wow, new revelation.

There's a big scar band
on the left side of the nose.

One of these surgeries
caused a scar band

that was sitting,
blocking her airway.

Look at that. That was inside.

That was blocking
the whole left side of the nose.

The only way to fix that
is cut it,

get the septum straight,
and then put what we call

a splint in between that area
for a week,

so that that tissue will not
grow back together again.

I went through
all those surgeries.

This is what I really wanted
from the beginning.

‐ Oh, wow. Hey.

‐ Yes, bitch.

That's called good breathing,

How it feel?

Like driving down a highway
on a Sunday morning.

You got your top down, you know,

when your hair is flying
in the wind.

Since my surgery,
my nose is wider,

and I have regular nostrils now.

No more straws. No more
value meal plastic surgery.

I woke up from the surgery
in so much pain.

I was screaming in pain, crying.
No one cared.

After surgery, you were
pretty much left alone.

I mean, you could've

I paid someone $150
to mess me up.

My eyes‐‐they were swelling,
and I freaked out.

I was like, Dear God,
please don't let me go blind.

International Disasters

Our number 10 patient

came to us after she flew
all the way to Turkey

for a result that was nothing
to laugh about.

Maybe instead of going
to Turkey,

you should've gone to chicken.

‐ Do you know what I mean?
‐ Oh, my God.

That was‐‐Get it?
Turkey, chicken?

And although Terry tried
to lighten the mood,

his horrible dad jokes
couldn't make her laugh.

I'm Jorden, I'm 24,
and I'm from London, England.

I had my BBL done in Turkey,

and I guess you could say I was
left with a cat‐ass‐trophe.

Get it? Cat‐ass‐trophe?
[ chuckles ]

So I had, um, a BBL
and VASER lipo.

‐ VASER lipo of the abdomen?
‐ Yeah, of everywhere.

And that fat was injected
into the buttock area.

‐ Yeah.
‐ Interesting.

The problem
with ultrasonic liposuction

is that it destroys the fat.

If you transfer
that destroyed fat,

the chances any of it
will live

in the buttock,
once transferred, is very low.

The surgeon said,
"What do you want?"

"I want, like,
Kim Kardashian's figure."

He's like, "Okay, no problem."

But my bum was definitely not
what I paid for.

My bum cheek was so misshaped.

There is this bulge
at the bottom.

It's literally like an "L"
and awkward.

And because of lipo,
my stomach is mishappen.

I woke up from surgery
in so much pain.

I was screaming in pain,
crying. No one cared.

After surgery, you were
pretty much left alone.

‐ You could've actually died.
‐ Yeah.

At this point, I just want
a normal‐looking bum.

Okay, so you do have
significant laxity

on the lower stomach,

and let's talk
about the buttock area.

You have some laxity
right here.

It could be excessive fluids,
excess fat.

Turkish coffee.
I have not got a clue.

The solution for laxity,
loose skin of the buttock,

is a "potential buttock lift."

‐ Yeah.
‐ The other procedure

is a tummy tuck.
The only way to do

these properly
is to two‐stage them.

You do one first,
and then you do the other.

Buttock surgery requires you
to lay flat.

An abdominal surgery
requires you

to lay in a flex position.

Go for it.

Hopefully,
by the time this is over,

you'll not only have
a good result,

you'll really appreciate
the level of my humor.

I'm finding
these fluid‐filled capsules

and dead fat throughout
Jorden's buttock area.

Everywhere that ultrasonic fat
was transferred,

it caused either fat necrosis
or a fluid‐filled cyst.

with this. Phase one done.

Today is my second surgery.
It's for my tummy,

and I feel
really, really excited.

Okay, so this is a pretty
standardized tummy tuck.

was well over a year ago.

Hardly any bruising.

I feel good about something

When I look back and see what
happened to my BBL in Turkey,

it literally felt like
an absolute nightmare.

Number 9 went international

and got an unknown substance
injected into her face.

I mean, look at these things.

So they're not designer bags.

No, they are not designer bags.

They don't go with a thing
in my closet.

They're totally cramping

RIP to my bags. RIP to them.

Myesha was not only
a really fun patient.

It gave the opportunity
for Paul and I

to operate together,

and for me to finally teach him
how to do surgery properly.

I went down to Mexico.

I paid someone $150
to mess me up.

We live in L. A.,
and beauty is everything.

You're looking
at all these gorgeous girls,

and their face is so clear,

and I had puffiness
under my eyes.

It was horrible because it's
like something on your face.

You can't hide it.

I read that you could get
Restylane shots,

which will take away
the puffiness, the lines.

My friend told me
about her cousin

who worked in Mexico.

She said, "Oh, he'll do you
for $150." I was sold.

Was it one syringe?
Was it two syringes? How much?

It was, like, in a little jar.
It was white.

‐ And he sucked it up?

It didn't come
in a pre‐made syringe?

No, it didn't.

Once we got home, I started
having a reaction to it.

They were red.
They were swelling,

and I just freaked out.

dear God, please don't let me
go blind.

After the swelling went down‐‐
bags.

They were humongous. It was like
someone kicked my butt.

Sham of a doctor's office.

Sham of a job.
Just sham, sham, sham.

Make a small, little incision
on the inside of your eyelid

and a small procedure to remove
the fat buildup

will significantly improve this.

I think you guys just gained
a new best friend.

Myesha's fat pads‐‐they're two,
three, four times larger

than most of the fat
that I've seen.

It actually looks like
the filler

that she had injected in Mexico

made her natural fat pads
worse

through an inflammatory process,

causing blood vessels to grow
into it

and the whole area
to swell up. Problem is solved.

‐ Nicely done.

I don't have my bags.
I'm looking gorgeous,

and I am so absolutely happy.

for plastic surgery.

I don't speak Spanish,

and I was trying to tell them
I was in pain.

I just started vomiting blood,

and I felt
I was gonna die there.

Number 8 went international

for a mommy makeover
and ended up butchered.

My stomach is disgusting.

Crystal's story
was a heartbreaking one.

She probably didn't even need
any plastic surgery

to begin with,
but after she was seen

by 20 different
plastic surgeons,

her case was deemed unfixable.

My name is Crystal.
I'm 29 years old,

and I'm from Houston, Texas.

I have a Christmas tree
on my stomach.

I had two pregnancies,
and I gained a lot of weight.

My husband David said that
he actually really liked

the way my body was,
but at that time,

my husband was, like,
cheating on me a lot.

I felt like I was going to die,

and I didn't see my kids
for four months

because I was, like, smelly.

I didn't want anybody to see me.

And you weren't
on antibiotics?

‐ No.

This is right up there as
one of the craziest stories

we've ever heard.

Crystal is extremely lucky
to be alive.

[ crying ]
I feel like a monster sometimes.

Like, before I wasn't for me.

It's for me now.

So the big question
is do you have enough skin

to get this tissue down here
without pulling up

the pubic skin
and distorting that area?

Mom, what's your gut tell you?

‐ I have a good feeling.

‐ So do I.
‐ I do, too.

We're now cutting off
the remaining blood supply

that will determine
whether or not

we have enough oxygen delivery
to keep this alive

or whether we're gonna suffer
the same fate.

after having surgery in Ecuador

Kathy's nose
and facelift reconstruction

was way more complicated than I
originally thought it would be.

She had some
of the thinnest tissue

I have ever worked with
for a facelift.

After eight hours
of high‐risk surgery,

I was really worried
about serious complications.

It boggles my mind how there are
no standards.

You would've been better off
going and having

your plastic surgery done
by a mechanic in South Central.

I'm Kathy, and I went to Ecuador
for plastic surgery.

Unfortunately, my neck and my
nose got lost in translation.

My nostrils were up,
and my neck‐‐

he actually put more wrinkles
on it.

I'm a former model
and event planner.

I first got into modeling
when I was 10,

and then I went on
to do print work

and fashion shows.

"Playboy" approached me.

Have to say I love Hef,
and he's a great guy,

and that's how I got involved
in the event business,

was actually selling tickets
to some of the parties

at the "Playboy" mansion.

Being an event planner,
you have to look good,

no matter what, and at 52,
I decided, all right,

it's time to fix this neck.

I went to Ecuador in 2007.

Kathy did a typical medical
tourism trip‐‐huge mistake.

‐ He did a facelift.
‐ Yeah.

And he‐‐I'm butchered
all behind my ears.

And then he looked at my nose,

and he said, "You have
a little ball on there.

Let me take it off."
It was horrible.

They started giving me aspirin.

After a neck lift
and after rhinoplasty?

Yes.

So two areas where
you're gonna bleed a lot.

She was fed aspirin
immediately after surgery.

That's what we tell patients

to stop a week
before their surgery

because it causes bleeding.

I kid you not.
The movie "The Exorcist"...

[ makes whooshing sound ]
I just started vomiting blood.

I had no idea
what I was getting.

I felt trapped, and I felt
I was gonna die there.

It's like they amputated
all the septum off.

The nose is skeletonized.

A lot of stuff has been removed
and looks like a skeleton.

I'd like to actually take
your nose from being here,

all the way down to here.

You do have
an aging neck here.

‐ Right.
‐ And there are some wrinkles.

So we're just looking here
at your neck lift that you have.

Our game plan is to basically
do a revision

lower face and neck lift.

One thing that we know for sure
is that Kathy

is in better hands with us
in Beverly Hills

than she was when she was
in Ecuador.

The doctor in Ecuador took too
much of septal cartilage out.

With this PDS plate,

it's gonna allow us
to elongate the nose.

I think that looks pretty good.

Okay, let's start
the facelift.

We're making sure that we're not
near any of the nerves

that control the face.

All the nerves are staring
right at me,

just even a little bit
of pressure

or traction
on one of these muscles

will knock out the movement
of her lip.

I'm probably not gonna sleep
that well tonight.

I do not check the nerves
of the face right after surgery

because we've given the patient
local anesthesia,

and that will knock out
the nerves.

I usually wait
for the next day.

Do me a favor. Smile.

Go like this. Move your nose.

Raise your eyebrows.

Close your eyes really tight.

I was very careful
around the nerves,

but now seeing that they all
worked right in front of me

makes me feel a lot better.
All that stress is gone.

It's been eight years

since I made the worst mistake
of my life

going to Ecuador to have
my nose and neck done.

I'm back, you guys!

I was throwing up everywhere.

There's always risks
when it comes to surgery.

That was the time
when I went on life support,

and my sister and my mom
were being prepped for death.

At the time I was an idiot.

I had had a seizure,
and I had went into a coma.

Unfortunately, our next patient
has no fear at all

international disaster

is a frequent flyer for surgery

tempting fate
for perpetual beauty.

love to push the limits,

but with every added procedure,

there is an increased risk
of a complication.

Liziane was traveling
anywhere and everywhere

to find surgeons who would
finally say yes

to more procedures.

But one surgeon's willingness
almost cost her everything.

and was sent to prison,

I found out not too long ago

there was this group of friends
that we had,

and one of them had been
calling me

"Pig Nose" behind my back.

So what you're saying is
you're completely,

completely self‐conscious,

Rodrigo country‐hopped

finding surgeons,
but the difference with Rodrigo,

he went to the alleged
number one country

for rhinoplasties.

The type of bacteria that I see
that eats cartilage

way in the nose
would be called MRSA,

Methicillin‐resistant
Staph aureus.

And that is most likely
what you have.

The fact that you've had
so much surgery in the past

that you've basically disrupted
so much of the blood supply

and then put a piece
of dead tissue in that,

that that caused the whole thing
to die.

I am very confident

lying there with the ice.

That's an automatic burn
after 15 minutes.

The doctor walks in. I was like,

"When is the anesthesiologist
gonna get here?"

And then he's like,
"There's no anesthesiologist.

"That's why we‐‐we got the ice.

Believe me, you're not
gonna feel anything."

And then when I felt something,
oh, my God. I just screamed.

I mean, my God,
it was like a horrific scream.

It just felt like
I was being butchered.

Dr. Ice was
your anesthesiologist.

Dr. Ice was my anesthesiologist.

‐ You're kidding me.
‐ I'm not kidding.

The problem with using ice
as an anesthetic agent

is that it kills the tissue
before you operate on it.

So he essentially was operating
through dead skin.

What we theoretically can do is
make an incision here

and stitch it down here
back where nature wants it.

And fingers crossed that
this heals beautifully, okay?
