Emergency doctor Christa Bell
has been caring and repairing

for Gold Coast locals
for nearly 20 years.

I first came to Gold Coast Hospital,
as it was in Southport, in 1996.

So I came out from the UK.

I've known a lot of people
for a long time.

Some days you can come to work

and you know that if you're feeling
a bit rubbish

you'll get at least
five hugs in the day.

You'll feel better by the end
of the day

and that's just from
your colleagues, you know.

People do really look
after each other.

And it is nice walking 'round the
place and seeing people you know,

and it's like, "Hi!"

I think it's a friendly hospital.

Krista has a second specialty
in paediatrics.

And being a self-confessed big kid
makes her the perfect doctor

to look after the hospital's
youngest patients.

Her partner in crime is senior nurse
and mum of two Simone Stephens.

They really are a dynamic duo.

I love caring for people.

And you can ask any of my friends,

it doesn't stop when
I walk out the door.

How long have we
worked together, Christa?

Forever, for a long time.

the first time I met you,

I looked after Mia
when she scalded?

My daughter came in, that was
the first time she burnt herself.

She was only six months old.

Six months old. And now she's 12?

She's nearly 13.

Over 12 years we've worked together

and we've had some interesting
shifts here and at the old place.

So we have an 88-year-old male,
conscious, breathing,.

These two often work together

and typically on their shifts
they don't get much time to relax.

I think any job there's good days
bad days, pros, cons.

You never know what your
next job's going to be.

So there's an element of
sort of excitement there

and it keeps you on your toes.

Um, he has history of emphysema.

Caller also states that he can hear
gurgling in his chest.

I started off doing
biomedical science first

and then transferred
straight out of school.

So, I didn't come straight
into paramedicine.

Knock-knock.

First stop tonight
is Frank and Betty.

Who were having a quiet night
watching the telly

when Frank got all breathless.

You're having a little difficulty
with breathing today?

Yeah.

It's getting worse now.

It's been bad
for a couple of hours.

Any other medical conditions?

Old age.

Nice, deep breaths for me.

So your heart rate's
a little bit faster.

(UNINTELLIGBLE)

Yeah, because of your breathing,
obviously,

and your blood pressure's
a little bit up

but you don't have any
temperature or anything

so this might be just the start
of an infection coming up

and with the weather going on,
you know,

with your emphysema,
it plays around a bit.

I try and laugh,
if you don't laugh, you cry.

And my old girl will be able to have

His wife's been telling him
for a while

that he needs to go see the doctor.

So, but some men can be
a bit stubborn, so...

the girls are already hearing
their next call.

Well, that really
tricky-looking aneurysm, isn't it?

Every day is a challenge for doctors
Hal Rice and Laetitia de Villiers.

And today is no exception.

As they attempt to repair
a swollen blood vessel

known as an aneurysm
in a young woman's brain.

It's getting to that stage

where it's becoming
very high risk for rupture.

They couldn't do it without a little
help from their colleague Michael.

Hal, Laetitia.

Hey, how are you?

Michael is our head radiographer

and he's been at
the Gold Coast Hospital

for as long as I can remember
and way before then.

And his main job is to keep
everybody fed.

(HAL:) Oh, my God.
Oh, yum.

You have been working up
late all night again.

He always brings in something great

that is baked or some interesting
food that he has discovered.

Go on, crack it open.
It's a caramel.

Look at that! That's going to
keep us going all day.

When he's not baking, Michael runs
the high-tech imaging machinery

that allows Hal and Laetitia to see
inside their patient's brains.

Alright, let's get going.
Back to reality.

Hey, how are you feeling?

Waiting for surgery is 22-year-old
Tarra whose problems began

when she was diagnosed with
a brain tumour seven years ago.

Being 16, I had no idea how to take
it and then it was like a week

later and I was in surgery because
they thought it might be malignant.

So they raced me into
surgery and it was...

it was an insane process.

What time is mum and dad coming?

Um, seven.

While she went through
the terrifying brain surgery,

Tarra's high school sweetheart,
Justin, stayed by her side.

Yeah, Justin is amazing.

You know, at 16 you don't think

that a guy like that's
going to stick around.

But he'd just sit with me
even if I was asleep,

I'd wake up and he was there.

It was the best feeling

so I know that this time
it's gonna be just the same.

So, what time are you going in?

Soonish.

Definitely brought us closer, if
anything, like being 15 years old,

you know, you sort of like, what's
going on, why is this happening?

And spending so much time
with Tarra in hospital

made me realise how much I love her
and we eventually got married

and bought a house, got a dog.

It's perfect.

But threatening to end the
newlyweds' domestic bliss

is a giant aneurysm that has formed
in Tarra's brain

where the tumour was cut away
seven years ago.

I feel like it's a time-bomb,
you know, in my head every day.

Just waiting.

It's a busy night for paramedics
Adele and Sophie.

They've already offloaded
one patient

at the Gold Coast
University Hospital

and are now heading back
to the city streets

under the light of a full moon.

No-one can prove it but every time
it feels like

it's always busy on a full moon.

More mental health

and everyone seems to get out
and drink and I don't know why.

It's a bit crazy.

And the girls' next job has a touch
of the full-moon madness about it.

We have a 49-year-old female,
cut finger with knife.

"Cut left index finger
while cutting avocado."

Says "spurting out."

"Bleeding control has been applied
but cut finger accidentally."

So, um...

Because it's come as a serious
haemorrhage, it is a Code 1.

Code one means
it's a time-critical case

where the patient
is in imminent danger.

What's your name, dear?

But Sophie and Adele
are not so sure.

There's the crime scene over there.

Yeah... Have a seat.

We got called Code 1,
which is lights and sirens

'cause it's serious haemorrhage,
blood squirting everywhere.

Obviously that's their perception,

our perception of blood loss
is quite more severe.

Yeah, it looks like

you've just nicked a little vain
there, on the side. So...

Yeah...

What we're going to do,
we'll wrap it up...

It's usually on the roof,
on the walls, all over the ground.

Drenched in it.

Drenched in blood.

But a lot of the public members,

their idea of blood squirting out
and blood loss

is really quite minor.

Yeah, you have nicked there.

I don't think I have nicked it,
I think I've smashed it.

It's slowed down.

It definitely needs some
stitches, OK?

Are you alright with the pain there?

I have had enough to drink
that it's...

That's probably why you're bleeding.

Caroline enjoyed
a glass or two of red

while making her salad but maybe
the avocado wasn't such a good idea.

So I'm on the bed?
Yeah.

Now she's relying on designated
driver Sophie

to get her to the hospital
for a couple of stitches.

I didn't even want
the bloody avocado.

(RADIO CHATTER)

Are you sure you don't want
pain relief

because we're happy
to give it to you.

I think she was a little bit
embarrassed

because it was a little nick

that she probably could've taken
care of herself

and come to hospital
on her own terms

but because she had alcohol

she can't really drive herself
to hospital.

I'm just going to park you

She sort of want of the ambulance

service to come and help
in her time of need.

She hasn't lost her finger tonight
so that is a good thing.

So, note to self,
maybe drink alcohol

after preparing dinner with a sharp
knife and not during or before.

Six-year-old Tommy has turned up
with a very unusual break.

He was leaping between benches
in the school playground

when he fell heavily
on his right arm.

I tripped, my foot got stuck
and I smashed my arm.

Oh, dear. Did you cry?

Yes.
Yeah.

But you're not crying now which
means you must be pretty brave.

I was crying for a long time,

it took a long time
to get to the hospital.

Did it?

As a mum of two,
senior nurse Simone Stephens

can't help getting involved
in looking after little Tommy.

I think, you and I, our favourite
is the children, isn't it?

Yes. Kids is our passion.

Yeah. Absolutely.

Making it all lovely and calm
and a nice experience

for these little kids who,
it's just rubbish, isn't it?

Breaking your arm but that kid's
pretty happy, isn't he?

You've been sucking on that green
whistle. Has that been helping?

Yep.

The green whistle contains a strong
painkiller that's safe for children.

Look, he's out of it.

He is just puffing on that.

He's having a good time

you'll get a bit.

Dr Christa examines Tommy
while his mum, Jackie, watches.

Don't freak out, Mum.

Can you straighten your fingers?

It's all about their pain,

you don't want them to be
in any pain.

So, Mum, just a temporary sling
so it's all supported.

It'll mean it's so much better when
we move him around for his x-ray.

I might confiscate is now, I don't
think you need any more of that.

Mum is going to have that.
He liked that.

Hello, Simone's phone,
Christa speaking.

A bad time to give up smoking.

(TOMMY:) Where's the whistle?

It's hiding.
Alright, we will have an x-ray.

I want the whistle.

Tommy gets the empty painkiller
whistle to keep him happy

while he's taken for an x-ray to see
exactly what's broken in that arm.

Can you stay nice
and still like a statue?

(TOMMY:) Ow!

Good boy.

For Tommy's mum and grandma,

the x-ray images confirmed
their worst fears.

It's real now.

So what time are you going in?

Soonish.

22-year-old Tarra has a giant
aneurysm growing in her brain.

I feel like it's a time-bomb,
you know, in my head every day.

Just waiting.

If the aneurysm bursts,
Tarra would need to within minutes.

There is no way of knowing,

if it's on the brink
of doing anything or not.

Left untreated,
that's highly likely.

(LAETITIA:) By aneurysm standards,
it's a big aneurysm.

It's about 14mm which is 1.5cm

So, anything over 7mm
is considered big.

And the bigger they get,

the higher their chance of rupturing

and causing a fatal
brain haemorrhage.

Thankfully, you know,
in a lot of these cases

when we get the patient in time,
we're able to make a big difference

to their lives and certainly
it's still a very young person,

they've got many years
of life ahead of them.

So, our responsibility
is to get them through

that threatening episode,

get them recovered and get them
back on to their life.

But the surgery itself
may also cause a brain haemorrhage.

It is scary but it's important
for her to know that

I am there and I am always going
to support her no matter what.

So...

Yeah, I'm feeling very nervous
about the surgery.

Because I know what I went
through the first time,

I think it's harder going into
this one

and knowing what's
going to be coming.

So...

Tarra and Justin's hopes
for a future together

are relying on
Hal and Laetitia's ability

to safely repair the giant aneurysm.

who's really done a job on his arm.

Tommy took a tumble
in the school playground

and managed to break both
of the bones in his forearm.

D'you reckon we'll be
able to do that here?

The bones in the arm will have to be
pulled straight by hand

while little Tommy is under sedation
to numb the pain.

Did you want me to get the ketamine
and everything ready?

No, I've got the ketamine.

Do you want me to check in with you?
So 100mg in 10ml.

With any medication with children

you just can't afford
to make a mistake.

I mean, you'd almost
triple check it.

Ketamine and just draw up
the first 1ml.

1ml and yep that will go into 10.
Thank you.

Being under the influence
of ketamine

can be a bit frightening
to a six-year-old.

So Tommy and mum Jackie
get some special attention.

You've got to de-escalate
the fear, remove the pain,

remove the anxiety from the parents.

And, for the child,

the experience you want it to be
nonthreatening, not frightening.

So, that's where we use our
procedural sedation room.

Our visualisation,
a little bit of magic medicine.

But Christa knows sometimes the mums
also need a little bit of magic

to help get them through.

Sometimes you have just got to say
"They know what they're doing

"and it looks bad to me
but they know what they are doing."

And... you just have to trust.

I'm just going to see Elise
and take her through.

Midwife Tionie is truly passionate
about helping women

go through pregnancy and childbirth.

It's a privilege and an honour
to be with women

when they're having their babies

and it really is all that it's
cracked up to be.

Working in the maternal-fetal
medicine department

means Tionie gets involved
in moments of great happiness

but also of great tragedy.

It's a department that exists
specifically for women

that have high-risk pregnancies.

We see anything from women

that have babies where we're
concerned about their growth

to more serious cases where there
are serious diagnoses

of babies that have chromosomal
abnormalities or foetal anomalies,

structural anomalies.

Some these babies won't survive.

There are many pregnancies that
don't end the way that we as parents

want them to end.

Losing a baby was
a life-changing experience

for young couple Shelley and Jai.

We had a baby last year
the in August.

But tragically he only
lived for one day.

Baby Leo had a heart defect
that was discovered

just hours before he was
delivered by emergency caesarean.

After we got home,
we were really lucky

to have captured quite a few photos.

In the hospital.

So I just put this book together.

I started with the photos in order
from when he was alive.

And then some of
the photos were taken

from a foundation called Heartfelt.

And they take professional
photos for you

in a time where your baby
is sick or passed away.

And then we've got a little bit on
the day that we said goodbye to him.

There's photos in here
of the candle,

Leo's candle and everyone
who attended

lit a small candle and placed it

next to the bigger candle.

We also shared planting
a garden for him.

I couldn't bear not to have
memories of him

and walk past him in the hallway.

You know, see his photo
on the wall or...

..wake up to seeing him
in his little urn to our bed.

Yeah... I...

love his memory,
so I love keeping it alive.

Tionie has been keeping
a close eye on Shelley

and her new baby
throughout this pregnancy.

(TIONIE:) How are you?
How you going?

Any pregnancy after a loss is
a difficult pregnancy.

And I don't think that it matters

how many pregnancies that woman has

because there's always that
worrying, the concern in their mind

that perhaps not everything is OK.

Relax, make the most of it.

(SHELLEY:) We got recommended
to have a scan

at 24 weeks because
that's the best time.

For you to see
the development of the heart

and that is the part that
they were most worried about

re-occurring with what
happened with Leo.

Tionie is doing everything she can
to create happy birth memories

for Shelley and Jai.

But until their new baby is safely
delivered, there are no guarantees.

High school sweethearts Justin
and Tarra have just been married.

But a large aneurysm in Tarra's
brain threatens their happiness.

So, an aneurysm is a weakness
in the blood vessel wall

and basically it's like
a ballooning out

on one side of the blood vessel wall

and the wall gets
thinner and thinner,

the larger the aneurysm gets.

If the thin wall
of the swollen vein bursts,

Tarra would probably die in minutes.

I feel like it's a time bomb,

you know, in my head every day,
just waiting.

X-rays on.

Neuro interventional radiologists
Hal and the Laetitia

will try and cut off the blood flow
to this grape-like bulge.

Nasty.

Yeah, it's a really
ugly looking aneurysm.

It looks like it's got a bit bigger.

The aneurysm has grown in size,
it's now 15mm.

It was about 13 previously.

So, it is certainly on the move.

The first stage of the repair
will be to fill the vein

with platinum wire coils

fed in through opened artery
in Tarra's groin.

So what I'm doing,

I'm just pushing the coil
into the micro catheter now.

And then Laetitia's gonna position
the coil within the aneurysm.

The idea is the coil
will clog up the aneurysm.

Obviously you've got to be
very gentle

when you are working in
the arteries in the brain.

They are very delicate,
you can easily tell them.

Patients that we treat have serious

life-threatening conditions
and a tiny wrong move can be fatal.

So it is very intense.

At the moment we are just trying to
navigate the wire into the aneurysm.

Just going around all
of these twisty corners.

A ruptured aneurysm wall
would cause a tragic death.

It's kind of hard to see where
the neck of her aneurysm is.

Laetitia is determined
to not let this happen.

Driven not only by profession
but also a deeply personal secret.

Six-year-old Tommy needs the bones
in his arm pulled straight.

So to numb the pain for the
orthopaedics to do their stuff,

he's been given
the heavy painkiller ketamine,

which will give him
an out-of-body experience.

♪ And watching the ships

♪ That go sailing... ♪

(WHISPERS) Mum's a good singer!

As little Tommy drifts off, Christa
makes sure he's not getting spooked.

How do you call for dolphins?

Have you got a magic
call that you can do?

(DOLPHIN CALL)

That's a funny noise they make,
isn't it?

Dolphins come and then are you gonna
ride on the back of the dolphin?

(TOMMY:) Yeah...

Awesome! That's so cool.

And you're going to go up
over huge big waves.

And the water splashing
all over your face.

Yeah.

Splashing water.

Yeah.

That's just 10mg.

(SIMONE:) That'll be the ketamine
taking effect.

The eyes were open
but it's a funny feeling for him.

It can be quite distressing
for parents to watch

because it's like,
"Hang on, that's not my child."

See how his eyes
are starting to go?

He's is probably
hallucinating right now.

Ketamine is an interesting drug.

It's like they go into this,
almost like a night terror,

so their eyes are open.

Their body will feel
all sort of different.

We try and make it nice and
quiet in there, we dim the lights,

do all that sort of stuff just
because they can get overstimulated.

It disturbs parents sometimes
'cause their eyes looking around

and you think they're awake

but he will have no recollection
of what we are about to do.

He is a brave little thing.

With Tommy now under, orthopaedics
can pull the broken bones straight.

That's what I said,
eyes are open but no-one's home.

(ORTHOPEDIST:)
Try and keep it at a 90.

It seems scary, you're right.

I'm fine.

Sure?

You want to sit down?

So, he may flinch a little bit but
he won't, he won't be aware of it.

OK.

(TENSE MUSIC)

Are you sure he's not feeling this?

Are you OK there?
Can you breathe?

I won't keep you there for too long.

I'm just going to have a feel
of your tummy, is that all right?

Midwife Tionie is checking
on Shelley's fully formed baby girl.

Who she's already named Ayla.

Most importantly,
Ayla has a strong heartbeat.

..which is beautiful.

Really happy with baby's heart rate.

It's very normal
and it's exactly what it should be.

All right?

It's great news for Shelley
and her husband, Jai,

who lost their first baby boy Leo

to a heart defect
just a day after he was born.

In Ayla's bedroom,
there is a framed picture

that has Leo's hand
and footprints on the wall.

As a memory of her big brother.

This is Leo's hands and feet.

So, yeah, this was something special
to be in her room. Big brother.

Looking over her.

A pregnancy after loss
is called a rainbow pregnancy.

A rainbow baby.

So there's a little rainbow
on her belly. Yeah.

Shelley's rainbow baby should help
heal some of the emotional scars

from when she lost baby Leo.

Jai did it. And we painted it. Yeah.

(TIONIE:) She is still having
a baby, it is still a birth for her

and we've got to do what we can

to create that really
special time for her.

It's not just about Shelley, it's
also about the dad, too.

I can't imagine how difficult
it was for you last time

having gone through
an emergency caesarean,

especially when you weren't there,
Shelley was put to sleep.

You didn't know
what was going on.

Didn't know if Shelley was OK,
didn't know if your baby was OK.

I'm not sure if it was just
in the rush

but I was told I was going to be
able to be present

so, I got kitted up
in all of the equipment.

Yep, ready to go into theatre.

Next minute
I'm not allowed to be in there

so, I want to be present this time.

You are giving me goosebumps, yeah.

Sometimes when we're going
throughout this next birth

and having this next baby,
memories can come up from last time.

You could start if you flashbacks
or you start to feel emotional

because of everything
that happened last time.

I want you to know that
that's normal and that's OK.

In four days' time,

Shelley and Jai's baby girl
will be delivered by caesarean.

It's an exciting but nervous wait.

Grief certainly affects
their next pregnancy

where there is always that

small part inside them where they're
just so scared that perhaps

this is another pregnancy and
another baby that might not make it.

It's kind of hard to see were
the neck of her aneurysm is.

Hal and Laetitia are in the middle
of a delicate operation

to fix an aneurysm
in a young woman's brain.

A condition that Laetitia
has personal experience with.

My youngest brother died
of a brain haemorrhage.

He was 16.

I was in my fourth year
of medical school.

Losing her brother
to a brain haemorrhage

inspired this brave doctor
to help others in danger

of suffering a similar fate.

It's really difficult
but at the same time

you can really empathise and you
know, it's terrible for any family.

You can see the coil,
we have specifically chosen

this size and shape
because of Tarra's aneurysm

and that is why those measurements
earlier were so critical.

You can see as Laetitia's forming
the coil,

you're seeing how
it's sitting beautifully

within the outer confines
of the aneurysm.

And this being the 12mm coil
will be the largest one

that we use in this aneurysm.

In the aneurysm, you start with
a really big-sized coil

which has a preformed shape

which exactly fits to the size
of the aneurysm.

And as you can see there,
the flow within the aneurysm

has been dramatically reduced
just with one coil.

A total of five coils are carefully
fed in from Tarra's groin,

up through her heart
and into her brain.

Pack the aneurysm
fuller and fuller with coils

until there's no blood going into
it anymore so it can't burst.

And then over time
the lining of the artery

will actually grow over
the base of that aneurysm

and heal it off completely.

I don't think we should try
and put anything more in.

No. Absolutely brilliant.

Part one done.

Yes.

That's looking good.

We've done the hard work
with the coiling of the aneurysm,

we've got that well secured so this
next step now is very important.

This is the reconstruction
of the artery with the stent.

It's a metal mesh tube and it
changes the direction of the flow.

I'm just twisting the wire now
to get us past the aneurysm

which is the next difficult part.

In most aneurysms the blood flow is
pumping straight into the aneurysm.

It is a preference to the artery,

so you have this really
high pressure blood

pumping into the aneurysm
all the time.

And if you put a stent
across the bottom of the aneurysm

and reinforce the artery wall,

you're changing the direction
of the flow of the blood

and it's going to the brain
where it should be going.

This is the tricky part
of the procedure

because you want to place the stent
in the exact right position.

You don't want to fall back

or be too far into the brain
than you want it to be.

In an ideal world,
the stent is fed in compressed

and worked into position
and then opened.

It's a tense time
for Hal and Laetitia

because this stent
is not responding.

That's really not wanting to open.

Clearly the stent is
not behaving itself as we want.

And we have no other option
but to remove it.

called a stent

inside an artery
in a young woman's head.

(HAL:) This is the second time now
on the second stent.

Almost begging the stent
to open for you.

The veins they have accessed
the artery through are so twisted

that the mechanism on their wire
tool just can't get the stent open.

It's not coming.
We will try another device.

of the stent is open

but the end of the stent here
that's what was concerning us,

was not opening.

The team have already filled an
aneurysm in 22-year-old Tarra's head

with platinum wire
to cut off the blood flow.

The job of the stent is to make sure
the nearby artery stays open.

It's opening behind the aneurysm
reasonably well.

It's just as we get to the top here,
getting the same scenario.

If the swollen blood vessels were
ruptured by the wire tools,

Tarra could die from
a brain haemorrhage.

It doesn't want to open.

(SIGHS)

It does not want to open.

Three hours into the procedure, Hal
decides the risk is just too great.

OK, let's call it.

The key thing really is to know
when to stop.

The blood vessels in the patient
are very fragile so I think

three attempts at the stent at this
point is worth stopping.

Let her blood vessel
relax and recover.

It's frustrating.

It's something that does
happen from time to time.

I must admit, it hasn't happened

for the last couple of years and
occasionally the anatomy defeats us.

The next step for Tarra
is to recover

from today's long procedure with
a bit of TLC from hubby, Justin.

We got all the aneurysm blocked off
nicely with the coils.

We wanted to put the stent in,
as you know,

nut we couldn't get it
to sit in nicely today.

So, certainly we have got a few
plans to have another attempt

whenever she is ready
and comfortable for that and because

we've actually fixed
the aneurysm already,

it will take the pressure off
that side of the procedure

and we can focus a lot more

on deploying the stent effectively
for her.

It's not dangerous
to leave things as they are,

all the serious stuff's over.

I know she's safe, she's well.
